Subtype Features And Clinical Significance Of Ki-67,bcl-6 Expression In Malignant Lymphoma Of Tonsil

The initial tonsillar biopsy was reexamed and additional sections for immunohistochemistry analysis were cut.50 cases were studied and immunohistochemical staining for CD56, TdT ,Ki-67 and bcl-6 were made.All the monoclonal antibodies were carried out with two-step method..To reclassify the subtypes of 50 cases of tonsillar malignant lymphoma and study the expression and significance of proliferation associated nuclear antigen Ki-67 and oncogene bcl-6 protein are why we have carried out the research.. According to the new classification of WHO(2001),we have obtained the following results 1. Histopathologic morphology presented as diffuse follicle centre lymphoma(66%,33/50),diffuse large B-celll ymphoma(18%,9/50),follicular lymphoma(8%,4/50),peripheral T-cell lymphoma(4%,2/50) and Burkitt's lymphoma(4%,2/50).2.The expression of Ki-67 is positive in 46%(23/49) malignant lymphoma of tonsil,47%(22/47) positive in B-cell lymphoma,50%(1/2) in T-cell lymphoma.FL 25%(1/4) ;FCL 45%(15/33);DLBCL 50%(4/8);BL 100% positive for it.Positive expression of Ki-67 showed no significant difference in sex,age,phenotype and grade of malignanacy of tumor (P>0.05). 3.The expression of oncogene bcl-6 is positive in 88%(44/50) of malignant lymphoma of tonsil , 84% (31/37) in FL(including follicular and diffuse) ;100% (9/9) in DLBCL;100% (2/2) in BL, Stastatics analysis showed no significant difference of bcl-6 expression in age,sex,phenotype and grade of malignanacy of tumor (P>0.05).4.Coexpression of bcl-2 and bcl-6 are positive in 56% (28/50) of all cases ,78% (7/9) in DLBCL;58% (19/33) in FCL;50% (2/4) in FL. Acording to our results,we have gotten many conclusions:1.All the cases of Tonsilar malignant lymphoma are NHL.2. B-cell immunophenotype are in 96% of them.T-cell leanage are 4%.3.The most histological subtypes is FCL,in turn DLBCL,BL and PTCL.4.The positive rate of Ki-67 antigen is 46% in our group.95%-100% of tumoral cells express Ki-67 in BL.5.The expression of bcl-6 vary in different subtypes such as FL,DLBCL,BL and PTCL.It's expression significantly higher in the agressive subtype than the indolent one.6.There is a rising tendency of bcl-2 and bcl-6 coexprssion in FL,FCL and DLBCL in sequence. We have summed up subtypes features of tonsillar malignant lymphoma and studied the significance of the expression of proliferation associated nuclear antigen and oncogene through our expriment .It is useful for further rcecogniation of the tumor,early diagnosis and prognosis .
