Objective To observe the changes of oxygen metabolism during orthotopic liver transplantation without veno-venons bypass. Methods 47 patients with end-stage hepatic disease were scheduled for OLT under combined general anesthesia; veno-venous bypass(VVB) was not applied to the cases during the anhepatic phase. Pulmonary artery catheter was inserted into the right jugular vein, an arterial line was put into the left radial artery. Hemoglobin(HGB), Blood temperature(TB), mean arterial blood pressur(MAP), cardiac output index(CI), arterial oxygen saturation(SaO2), mixed venous oxygen saturation (SvO2) , oxygen delivery (DO2) ,oxygen consumption(VO2)and oxygen extraction ratio(O2ER)were measured during perioperation.Result There were no significant changes in HGB ,SaO2 . Compared with those during preohepatic phase, CI, MAP,VO2,DO2 decreased significantly during anhepatic phase(P<0.01) and thenincreased gradually during the early reperfusion stage to the level ofpreohepatic phase . ERO2 increased significantly during anhepaticphase(P<0.01) and decreased to the level of preohepatic phaseduring neohepatic procedure. Conclusion The oxygen delivery and oxygen consumption changedsignificantly during OLT(NON-VVB).
