Research background and Object: Hepatic ischemia-reperfusion injury is one of the main cause which induces the graft dysfunction during operation or post-operation of liver transplantation. The mechanism of the liver ischemia-reperfusion injury is so complicated, it involve in so many reasons, such as liver parenchyma ischemia and hypoxia, over production of oxygen free radicals, inflammatory cellular factors release, intracellular calcium overload, microcirculation dysfunction, energy exhaust, and so on, but oxygen free radicals over production and inflammatory factors release are the most important causes. Oxygen free radicals aggressive production during the liver ischemia-reperfusion injury may be is the earliest and most main cause that induce the change of structure and dysfunction of liver graft. During the anhepatic phase of orthotopic liver transplantation, inferior vena cava and portal vein are totally or partially obstructed that will cause congestion in viscera and lower limbs, and then product much metabolite including oxygen free radicals and endotoxin that will enter the systemic circulation at the neohepatic phase, and the acute inflammation caused by liver ischemia-reperfusion injury will make organism severe injury, all of these problems are need to be resolved in liver transplantation. So reducing the production of oxygen free radicals and reasonable using the oxygen free radicals scavengers may be the valuable way to protect liver from ischemia-reperfusion injury.
