Objectives: To examine the expression of some cell adhesion molecules and proteases associated with tumor invasion and their relation with clinicopathological parameters and survival rate in synovial sarcoma, and provide valid index and academic basis for clinical therapy and prognosis.Methods: Immunohistochemistry for E-cadherin, |3 -catenin , CD44v6, MMP-2, MMP-9 and cathepsin D were performed in a tissue chip including 72 cases of synovial sarcoma. The positive expression rates and labeling indexes of E-cadherin, P -catenin , CD44v6, MMP-2, MMP-9 and cathepsin D, and the abnormal expression rate of# -catenin were investigated and compared with clinicopathological parameters and survival rate. Results:1. Expression of cell adhesion molecules and their relation with clinicopathological parameters and survival rate in synovial sarcoma 1) Expression of E-cadherin, # -catenin , CD44v6 in synovial sarcomaE-cadherin and CD44v6 were mainly distributed in cytomembrane of epithelioid cells and expressed little in spindle cells that were bigger and rounder than typical spindle cells. Three expression patterns of # -catenin were observed in synovial sarcoma, only in cytomembrane, only in nucleolus or cytoplasm, or in cytomembrane, nucleolus and cytoplasm. The first pattern was positive expression and mainly located in cytomembrane of epithelioid cells. The two latter patterns were abnormal expression and mainly in spindle cells.Positive expression rates of E-cadherin, P -catenin and CD44v6 were 25.00%(18/72), 34.70%(25/72) and 9.70%(7/72) respectively, and labeling indexes were 6.85%> 24.54% and 4.94%. The abnormal expression rate of P -catenin was 45.83%(33/72).And positive correlations among positive expression rates of E-cadherin, P -catenin and CD44v6 were significantly showed (P<0.05 for all). Positive correlations among labeling indexes of E-cadherin, P -catenin and CD44v6 were significantly showed (PO.05 for all),too.2) Correlation between the expression of E-cadherin, P -catenin , CD44v6 and clinicopathological parameters in synovial sarcoma(D The positive expression rates and labeling indexes of E-cadherin, P -catenin , CD44v6 in biphasic type were significantly higher than those in monophasic fibrous type (PO.05 for all). The abnormal expression rate of P -catenin in biphasic type was significantly lower than that in monophasic fibrous type (P=0.01).It was suggested that the positive expression of cell adhesion molecules was mainly located in biphasic type.(2) The positive expression rate of CD44v6 in primary group was significantly higher than that in reoccur or metastasis group (,P=0.03). The positive expression rates of E-cadherin and P -catenin in primary group were higher than those in recurrence or metastasis group, but which were not significant. The abnormal expression rate of P -catenin in primary group was significantly lower than that in recurrence or metastasis group (P=0.01). These results suggested cell adhesion molecules were lowered in recurrent or metastatic synovial sarcoma.(3) Negative correlation between labeling index of E-cadherin and tumor grade was showed significantly (PO.OO), while no significant correlation6was found in other parameters.?Negative correlation between labeling index of E-cadherin and tumor phase was showed significantly (P=0.01), while no significant correlation was found in other parameters.(5) Positive correlations between the positive expression of E-cadherin and Cytokeratin, between the labeling indexes of E-cadherin and Cytokeratin, between the labeling indexes of CD44v6 and Cytokeratin were showed significantly (P=0.02, P=Q. 000, P=0. 047 respectively). No significant correlation between P -catenin and Cytokeratin was observed. 3) Correlation between the expression of E-cadherin, P -catenin , CD44v6 and survival rate in synovial sarcomaSurvival with E-cadherin positive expression was better than with negative expression (P=0.05).
