Objection: This study was to investigate distribution of abnormalities and lesions underlying of Electrophysiologal abnormalities on the nerve fibres of distal and proximal on limb, the nerve fibres of superior and inferior limb , and the nerve fibres of both sides. Compared abnormalities of conduction and lesions degree of nerve conduction velocity (NCV) between AIDP groups and AMAN groups .The studies would be significant to offer the objective indications for diagnosing, classifing the subtypes, evaluating Prognosis of GBS. Methods: ⑴ 51 cases of GBS were divided into AIDP group and AMAN group.⑵ 51 cases were comprehensively examined using by electrophysiological techniques.⑶ Observed the appearance of the fibrillation potential and positive sharp wave. ⑷ Recorded distal motor latency(DML), motor conduction velocity(MCV), compound muscle action potential(CMAP);and latency, sensor conduction velocity(SCV), sensory nerve action potentials(SNAP) amplitude. ⑸ All datas were dealed with by statistics Analyse with SAS software.⑹ Compared the abnormal conduction in motor and sensory nerve fibres, distribution of abnormalities and lesions degree of the superior and inferior nerve fibres on limb, the distal and proximal nerve fibres on limb , both sides, and the electrophysiology difference between AIDP group and AMAN group. Results : 1 AIDP group ⑴ EMG Having examined 156 pieces of muscles, 101 appeared fibrillation potential and positive sharp wave, We compared distal muscles(79.2%) to proximal muscles(50%) p<0.05, and muscles of superior limb (64.4%) to muscles of inferior limb(71.2%),p>0.05,there were no statistical significance.⑵ Compared MCV(78.2%)to SCV(64.1%), p<0.05,the difference was significent. ⑶ Compared to the healthy controll group ,the date of peripheral nervous were abnormal with DML prolonged , NCV slowed down, and Amp of CMAP lowed,p<0.05. there were no statistical significance(p>.05) on both sides.⑷ Abnormal distribution of DML:Compared the rate of DML's abnormalities of proximal nerves 74.3% to that of distal nerves 82.2%, P>0.05, the rate of DML's abnormalities of proximal nerves (75.2%)to distal nerves(76.9%)on superior limb, P>0.05, the rate of DML's abnormalities of proximal nerves73.1% to distal nerves 92.3% on inferior limb,P>0.05, there were no statistical significance. Compared the rate of DML's abnormalities of proximal nerves on superior limb75.2% to that of on inferior limb73.1%, P>0.05, the rate of DML's abnormalities of distal nerves on superior limb (76.9%) to that of on inferior limb(92.3%), P>0.05,there were no statistical significance. That showed abnormalities distributing generally symmetric on limb, but the abnormalities of common peroneal nerve was the highest among examined nerves. The severity of DML: The rate of DML's severe lesions of distal nerves(75.6%)was higher than that of on proximal nerves(47.4%), p<0.01, there was statistical significance. The rate of DML's severe lesions of distal nerves on superior limb(71.2%)was higher than that of proximal nerves(51.9%), p<0.05, and the rate of DML's severe lesions of distal nerves on inferior limb(84.6%) was higher than that of on proximal nerves(38.5%),p<0.01,there was statistical significance.Compared the rate of DML's severe lesions of Distal nerves(84.6%) on inferior limb to Distal nerves (71.2%)on superior limb, P>0.05;and the rate of DML's severe lesions of proximal nerves(38.5%) on inferior limb to proximal nerves(51.9%) on superior limb,P>0.05, there were no statistical significance, the degree of lesions was similar.⑸ Abnormal distribution of CMAP: Compared the rate of CMAP's abnormalities of proximal nerves 75.6% to that of distal nerves 82.1%, P>0.05, the rate of CMAP's abnormalities of proximal nerves (75 %)to that of distal nerves(76.9%)on superior limb, P>0.05, the rate of CMAP's abnormalities of proximal nerves (76...
