Local Application Of Fibrin Sealant Carried With Dexamethasone Preventing The Stenosis At Small Artery Anastomosis

Objective To investigate the efficacy of preventing the stenosis after the application of fibrin sealant carried with dexamethasone outer of small artery anastomosis. Methonds 24Wister rat femoral were cut and divided into 3groups randomly. Group 1 consisted of animals with conventional suture anatomosis (n=8). Group 2 consisted of animals with suture assisted FS anastomosis. Group 3 consisted of animals with suture assisted FS/DEXA anastomosis. FS/DEXA composite were prepared and been distributed on the outer surface of rat femoral anastomosis . the animals were sacrified 3 weeks after operation . Defined segments of artery were sectioned and observed under microscope and examined by using a immunhistochemistry . The image was digitalized by computer video analysis system . the intimal thickness and medial thickness were calculated and analyzed statistically among groups . Results Transmission electromicrope shows that the type of vascular smooth cells (VSMCs) are synthetic type in group1 and group2. they are shrink type in group3. The immunohistochemistry staining of PCNA(proliferation cell nuclear antigen) shows the hyperplasia of vascular smooth muscle cells of group3 were less than that of the control group1 and group2. The intimal thickness and medial thickness of group3 had decreased significantly in contrast to group1 and group2. Conclusion FS/DEXA can suppress the hyperplasia and migriation of VSMC and prevent the stenosis postoperative artery anastomosis. The investigation provides the early clinical trials assessing the preventable potential of stenosis after artery anastomosis . Fibrin sealant assisted anastomosis can decrease operating time and the bleeding at the anastomosis sites .
