Objective Bacteria infections is the main factor which lead to pupal necrosis and periapical periodontitis .Getting rid of bacteria and lipopolysaccharide in the root canal is very important to the prognosis of periapical periodontitis . 60 teeth which got pupal necrosis or periapical periodontitis were choosed to test appearance rate and CFU of bacteria dominated . Comparing the clinical efficancy and bacteria diference beteen calcium hydroxide [Ca(0H)2] and camphorated paramonochlorophenol (CMCP) , to evaluate Ca(0H)2 as an intracanal medication . Methods 60 permanent teeth which got pupal necrosis or periapical periodontitis were divided into2 groups randomly , Ca(0H)2 group and CMCP group. Record clinical symptom . After routine root canal preparation , samples of periapical were obtained and sent to lab for bacterial study ,root canal filled with Ca(0H)2 paste or CMCP paper point. After 7 days , record clinical symptom and date of bacterial study . Results 1, Mixed polymicrobial infection were found in the root canal of these 60 teeth .Anaerobes dominated , which appearance rate is 95% .2 , After 7 days ,clinical efficancy beteen two groups : better efficancy of Ca(OH), group is 83.33% , efficancy is 16.67% ; while CMCP group is 56.67% and 30% ,none efficancy is 13.33% Statistically significant difference of better efficancy rate beteen Ca(0H)2 and CMCP groups was found . 3, Bacterial study indicates : after treatment , appearance rate of mixed bacteria decrease to 20% in Ca(0H)2 group ,aerobes, staphylococcus, lactbacillus , actinomyus decrease to 0 , and anaerobe , streptococci , black pigmented bacteroide , neisseria , veillonela decrease 80% , 73.3% , 70%, 30% , 23.3% , but statistically significant difference of anaerobe streptococci . lactbacillus was found . While caculating CFU of neisseria , veillonela , actinomyus , black pigmented bacteroide , statistically significant difference of neisseria , actinomyus , black pigmented bacteroide was found (P<0.01) . Conclusion : This research shows Ca(0H)2 is superior to CMCP as an intracanal medication , it is worth for widespread adoption .
