Objective:Hepatic lipase (HL)play an important role in lipid metabolism, the abnormal of serum HL activity is closely related with dyslipidemia in type 2 diabetes mellitus(DM)Patients with DM2 and normals were studied,their fasting and postprandial levels of plasma glucose (PG),serum insulin (Ins),blood lipid (includes TG,TC,HDL-C,LDL-C) ,HL activity were measured. HL activity was measured by spectrophotometer. The levels of blood pressure (BP) ,body mass index (BMI),hemoglobinA1c (HbA1c) et al were also measured. We also investigated the relationship among PG,Ins ,TG,TC,HDL-C,LDL-C,HL activity and the possible correlation between every one of them in lrder to find the rule of blood lipid changes. So that we can study the significance of them in the pathogenesis of greater vascular disease with type 2 diabetes. Methods: 58 patients with type 2 DM (1999 WHO diagnostic standards), 30 male and 28 female, 59.1±13.9 years old. None had drinking or smoking and their function of liver and kidney are normal. All of them don't had medicines that infect the metabolism of blood lipid. Taken 16 persons as normal controls,10 female and 6 female, 58±11.6 years old. The blood samples were obtained from fasting and postprandial in OGTT test. We measured the levels of PG,TG,TC,HDL-C,LDL-C,HL activity and Ins,HbA1c et al, furthermore, compared them with normal controls. All members'BP,BMI,WHR,Homa-IR were also measured. We analyzed all the data in SPSS 11.0 softwarw. Continuous variables with normal distributions are expressed as mean (S.D). The comparisons between groups were done using Student's t-test for independent samples or one-way analysis variance (ANOVA), correlation and multiple liner regression were applied when appropriate. Logistic regression was used in order to evaluate the danger of macrovascular disease with type 2 DM. P value of<0.05 was considered statistical significant. Results: 1 The HDL concentration decreased significantly after the 75-g glucose tolerance(p<0.05), while the serum HL activity increased remarkably ( p<0.01 ) . Plasma TG concentrations were positively related to Homa-IR(p<0.05), and plasma HDL-C concentrations were negatively related to Homa-IR(p<0.05). 2 When we divided the DM patients into A(HbA1c<6%),B (HbA1c 6-8%),C(HbA1c >8%)groups by HbA1c, we found the area under triglyceride curve(TG-AUC) became gradually higher and the area under high dense lipid cholesterol curve(HDL-AUC) became gradually lowe(rp<0.05).there wereno significant difference in TC-AUC,LDL-AUC( p>0.05). The rate of macrovascular was difference in different groups. 3 When we divided the DM patients into different groups by the subtraction of glucose in OGTT test, we found when the subtraction became higher and higher, the HDL-C concentrations decreased(p<0.05)morer and morer and the HL activity increased(p<0.05).there were no difference in TG,TC,LDL-C( p>0.05). 4 When we divided all members into N(normal control),I (no greater vascular disease),II (have greater vascular disease) groups, the TG concentration,TG-AUC became gradually higher and the HDL-C concentration,HDL-AUC became gradually lower(p<0.05). there were no difference in TC,TC-AUC,LDL-C,LDL-AUC( p>0.05).
