Background and Purpose:Intestinal anastomosis leakage has been one of the most serious complications in gastrointestinal surgery, and it is difficult to treat. It is reported that anastomotic leak rates are range from 0% to 17.4%. Its occurance increases the morbidity and mortality rate greatly..For many years, scholars in and out of our country have been devoting to the biologic process research of anastomosis healing in hope of finding an effective way to help heal the intestinal leakage, have made many clinical and experimental research works, concluded various reasons that would result in intestinal anastomosis leakage. Although there are many advancements .There is not yet to this day a specific treatment .So this problem has not been fundamentally resolved. There are many development in chemistry, physical and operation technology, but research on the influence of regulating the peristalsis in intestine anastomosis leakage treatment has not been reported so far.Amomun Villosum is a kind of traditional Chinese herbal medicine. It is used for Spleen qi-deficiency in Traditional Chinese Medicine(TCM). As modern medical research shows that seeds extraction fluid of Amomun Villosum lour, can obviously promote the rhythmical movement of guinea pig's isolated intestinum ileum, enhancing the contraction amplitude andcontraction frequency .Zhu Jinzhao et al experimental research shows that Amomun Villosum can also obviously motivate the rat's gastrointestinal tract movement .Propantheline is one of the anticholinergic medicine that can selectively inhibit the movement of gastrointestinal tract smooth muscles. So it can be concluded that the Propantheline can antagonize the movement of gastrointestinal tract. The target organ of both medicines is the same, but their mechanism is on the opposite.Some peristalsis medicine is helpful for the recovery of intestinal function for post-abdominal operation patients, releasing the abdominal distension, nausea, vomiting syndrome, reducing the intestinal toxin absorption in critically ill patients. So can it be useful for anastomosis healing by promoting or on the opposite inhibiting the peristalsis? What is the mechanism?In this study, the experiment was randomized, placebo-controlled. According to pharmacological function character of Amomun Villosum and Propantheline, this animal experiment research is to observe the influence on intestinal anastomosis healing and to study its functional mechanism.Material and Methods:90 Sprague- Dawley (S-D) rats, fed with the standard of cleanliness class in the animal center of Zhejiang Chinese Traditional Medical College , weighing 250~320g, were divided into three groups randomly (A, B, and C group). There were 30 rats in every group. A group was taken as normal control group; B group was taken as Amomun Villosum group and C group was taken as Propantheline group. All rats fasted for 12 hours before the experiment, but are not forbidden of drinking. Each rat was injected Ketamine Hydrochloride into abdominal cavity with the dosage of 100mg/kg. And then abdomen was opened, the distal ileum was severed 15cm proximal to the ileocecal part, continuity was restored by an end-to-end anastomosis ,using one layer of interrupted inverting sutures with 5-0 silk sutures. After operation ,each rat in group Awas given 0.9% sodium chloride with the dosage of 10ml one time, twice a day. The group B and C was given the same deal with Amomun Villosum (0.25g/kg) and Propantheline (3.5mg/kg).On days 3, 5,and 7, 10 rats in each group were killed by cervical dislocation and then abdomen was opened. A 5cm long segment (located proximally to the lesion) was removed for determination of anastomotic bursting pressure . Then the 5 cm anastomotic segment, consisting of a measured 0.25 cm intestine on either side of the suture ,was excised and stored at -20°C for hydroxyproline assay.
