Chronic refractory wound is a common disease, that has become the focus in clinical research..The etiology of chronic refractory wound is usually high blood sugar, high blood lipids, immunological abnormalities and infection caused by systemic or local factors.Currently, the researches on the disease are more focuse on the stem cells, skin grafting, and skin of tissue engineering and so on. However, Using exogenous growth factor is too expensive.Its relationship with cancer development and the parts of application need further study. Surgical treatment also has its limitations. The Sheng Ji Yu Hong Plaster from thre《Wai Ke Zheng Zong》has been held in esteem by medicine surgery and traumatology, as the representative of "HuoXueShengJi". The modern research prove that it can anti-clotting, improve local micro-circulation and promote the effectiveness of granulation.Because of its good biocompatibility, collagen was selected as commonly used in biological material support. The biggest advantage of modified collagen is larger bore and stout collagen fibers. Comparing to the not modified, the collagen is more stable. In this study, it will inject the Sheng Ji Yu Hong liquid into the modified gelatin sponge. Applied to the rats and clinical ulcers to observe the influences of the wound.Objective This study was designed by animal and clinical experiments to explore the use of Sheng Ji Yu Hong gelatin sponge to repair wounds. The object is to find a better ways for clinical treatment of refractory wound.Methods SD 40 rats were divided into four groups, according to the principles of randomized control groups. Respectively, Vaseline gauze (F) group, Sheng Ji Yu Hong Ointment (S) group, gelatin sponge (M) group, Sheng Ji Yu Hong-gelatin sponge (SM Group).20 wounds were in each group. After modeling, in the 3,7,14d, using image analysis software was measured in the wound area. Wound healing rate calculated at different times. Observed the changes in the course of treatment, such as local wound inflammatory response, and the function of important organs, microscopic observation of morphological changes in wound granulation. By MPAIS200 imaging analysis system for vessel count; cyanide methemoglobin assay of hemoglobin content calculation; immunohisto chemical determination of b-FGF, TGF-β. The data were used SPSS software for statistical analysis. The second part of clinical study collected 20 patients with chronic ulcers. Applied score sheet to evaluate the influences of the patients who used the Sheng Ji Yu Hong-gelatin sponge.Result 1.SM group wound healing rate higher than S group and M group, the result was statistically significant P<0.05. S group and M group were higher than F group, P<0.01.2. Granulation microvascular of SM, S, M group was higher than the F group, the result was statistically significant P<0.01; SM, S group higher than M group P<0.01. The 14d, SM group and S group had no significant statistical difference P=0.38.3. The hemoglobin in each group were higher than those of F group, the result was statistically significant P<0.001. S group and M group was no significant difference.4. The 3d and 7d, TGF-βpositive cells of SM was significantly higher than the other groups, P <0.05. The 14d, the TGF-βpositive cells of four groups were decreased,and they were not significant difference.5. The 3d and 7d, bFGF-positive cells of SM was significantly higher than the S group and M group, P<0.05. The 14d, SM and S group were significantly lower than the F group. M group and F group was no significant difference P> 0.05.6. In the clinical trial,20 patients with ulcers, the total effective rate was 100%, of which 85% is markedly; and 15% is effective.Conclusion The healing time of Sheng Ji Yu hong gelatin sponge was significantly short, and improve the microcirculation of the wound. There is the possibility of reducing the scar, and provides a new way for the treatment of refractory wound.
