Gallbladder cholesterol gallstone is a common disease. Yeas of clinical and animal reserach has indicated that, the main pathophysiological change of gallbladder cholesterol gallstone formation is biliary cholesterol hypersecretion, which is related to lipid metabolism, the change of serum lipid, lipoprotein and apolipoprotein levels may have been involved in the formation of the gallstone. The study on serum total cholesterol, triglyceride, high density lipoprotein, low density lipoprotein and the levels of some apolipoproteins gives us a chance to discuss the characteristic representation of gallbladder cholesterol gallstone in serum lipid, lipoprotein, apolipoprotein and its prognostic factor, and may suggest new approach to the prevention and therapy of this disease.Leptin is the product of obese gene, recent animal studies discovered that it has the effect to increase biliary cholesterol secretion, influence kinetic factors related to cholesterol nucleation, gallbladdercontractility, or mucin secretion, which are all the key factors affecting important procedures in gallbladder cholesterol gallstone formation. Therefore, leptin may have been involved in gallbladder cholesterol gallstone formation and has an important role, and its level may be used as an indication to estimate susceptibility of cholesterol gallstone. However, there has been few clinical studies on the correlation of serum leptin and cholesterol gallstone, and the correlation between serum leptin and serum lipid, lipoprotein, apolipoprotein.ObjectMeasure the serum levels of leptin, lipid, lipoprotein, apolipoprotein of gallbladder cholesterol gallstone patients and health control, and study the correlation between serum leptin and serum lipid, lipoprotein, apolipoprotein. Discuss the characteristics of serum lipid, lipoprotein, apolipoprotein, leptin and the mechanism of gallbladder cholesterol gallstone formation, evaluate the value of measuring serum leptin levels and other parameters for gallbladder cholesterol gallstone patients.Methods and materialsA total of 39 gallbladder cholesterol gallstone patients are selected as gallbladder cholesterol gallstone group with another 25 healthy people as the normal control group. Patients of gallbladder cholesterol gallstone group are without bile gallstone, and gallbladder gallstones are identified as cholesterol gallstones with cholesterol content greater than 70% for component analysis using ultrared spectrum method; Two groups of objects are without cardiac, pulmonary, renal, hepatic and endocrine diseases, and there is no significant difference in age andgender. Measured parameters include gender, age, (body?) height, body weight, body mass index, serum total cholesterol, triglyceride, high density lipoprotein, low density lipoprotein, apolipoprotein AI, apolipoprotein B, apolipoprotein E and serum leptin level. Serum lipid, lipoprotein and apolipoprotein levels were detected by automatic analyzer, while leptin is measured using radioimmunoassay. The data are processed by SPSS 10.0 for statistical analysis, with P<0.05 considered statistically different.ResultsSerum HDL and LDL are lower in gallbladder cholesterol gallstone group than in healthy controls, while serum Apo B-100 is higher and leptin levels significant higher in cholesterol gallstone group than in healthy controls. There are no significant differences of serum triglyceride, total cholesterol, Apo-AI, Apo~E between the two groups. Within the cholesterol gallstone group, serum Apo-AI levels are higher of female than male , but no significant difference for serum triglyceride, total cholesterol, HDL, LDL, Apo-B and Apo~E. Within the healthy controls, the serum leptin levels are higher in female than in male, and there are no significant differences in all other parameters. In both groups, serum leptin is positively correlated to BMI, and serum leptin level in gallbladder cholesterol gallstone group is higher than healthy controls.
