PrefaceThere has been a recent explosion of interest in the notion that chronic low - grade inflammation and activation of the innate immune system are closely involved in the pathogenesis of type 2 diabetes. Studies have shown that circulating markers of inflammation, acute - phase reactants or interleukin ( IL) - 6 (the major cytokine mediator of the acute - phase response) are strong predictors of the development of type 2 diabetes. Although it is well established that insulin resistance and impaired insulin secretion are central to the pathogenesis of type 2 diabetes, it has been unclear how these abnormalities arise and how they are related to the many different clinical and biochemical features common in type 2 diabetes. Activation of innate immunity provides a new model for the pathogenesis of type 2 diabetes and the metabolic syndrome, which may explain some or all of these features, and points to research directions that might result in new therapeutic approaches for managing and predictive type 2 diabetes and its complications. The purpose of the research is to discuss the relationship between CRP, WBC as markers of subclinical inflammation and type 2 diabetes and diabetes with stroke.MATERAL AND METHODTwo hundred and forty subjects were studied in this program . The subjects were divided into three groups : team A is healthy control group , team B isT2DM group , team C is T2DM with stroke group . Common characters were recorded such as sex , age , duration , hypertension and smoking status . The subjects ate just they did as usual . All subjects were measured the. height , weight , BMI , fasting plasma glucose , fasting TG , fasting TC , HDL - C , LDL - C , fasting insulin level , WBC count and CRP level . Statistics data was conducted by SPSS 10. 0 program . Differences among the groups were tested by ANOVA . Linear regression were used to analyze different clinical factors . Multivariate regression techniques were used to test the hypothesis.RESULTSCompared with control subjects , the team B had significantly higher WBC count and CRP level , the team C had the same results ( P <0. 01) . The CRP level showed a significant positive relationship with WBC ( r = 0. 60 P < 0. 001), Age( r=0. 58 p<0.001), BMI( r =0. 14 p <0.05 ) , SBP(r=0.31 p< 0.001) , TC(r=0.43 p<0.001) , LDL-C (r=0.52 p<0.001) , HOMA -IR(r=0.51 p<0.001) , FPG(r=0. 53 p<0.001), FINS(r=0. 38 p<0. 001). Multivariate regression resulted in the conclusion that a significant correlation existed among the CRP level and WBC count, HOMA - IR, TC.The WBC count showed a significant positive relationship with CRP( r=0. 61 P<0.001) , Age(r=0.34 p<0.001) , BMI(r=0. 16 p<0.05) , SBP(r= 0.17 p<0.01),TC(r = 0. 21 p<0.05) , LDL - C( r =0. 33 p <0. 001) , HOMA-IR(r=0.30 p<0.01), FPG(r=0.27 p<0.01) , FINS(r=0.27 p<0. 001). Multivariate regression resulted in the conclusion that a significant correlation existed among the WBC count and CRP, LDL - C, TC.CONCLUSION1. CRP level,WBC count are significantly elevated in subjects with T2DM and T2DM with stroke compared with control group which proves T2DM is associated with low - grade inflammation.2.
