Objective To determine the expression of heat shock protein 72 in leukocytes from early acute trauma patients , to study the level and evaluate its relationship with the survival. Methods Thirty acute trauma patients ( APACHEIII>18) from ICU randomly were included as study subjects. Blood samples were serially obtained at three time points: days 0-1,days 2-6 and days 7—14 (also was the first period, the second period and the third period)after the trauma event. We separated leukocytes and measured expression of HSP72 by imrnunohistochemistry and evaluated the expression of HSP72 mRNA by semi-quantified RT-PCR , find out the peak point of expression and the relationship between its level and the survival (survival period>2 months) was analyzed.Results (1)The HSP72 mRNA could been detected during days 0-14 after acute trauma. There was significant statistic difference on the HSP72 mRNA level between the second period and the first period( p=0.00180.01), The same was the second period and the third period( p=0.0021<0.01). (2)The level of HSP72 mRNA in survival groups was higher than that in dead groups, There was significant statistic difference during the second period(F=7.878, p<0.01), There was statistic difference during third period(F=4.260, p<0.05), But There was not statistic difference during the third period. (3)For the high score and low score groups, There was not statistic difference within periods (first period F=0.155, p=0.697>0.05; second period F=0.025, p=0.874>0.05; third period F=0.243, p=0.626>0.05). (4)The strong positive rate of HSP72 expression was 10% (3/30) during days 0-1, The strong positive rate of HSP72 expression was 63.33% (16/30) during days 2-6, The strong positive rate of HSP72 expression was 23.33%(7/30), There was significant statistic difference during three periods( x~2=9.479, p=0.045<0.05).Conclusion The study suggests that the acute trauma patients can lead to a marked elevation of HSP72 and its peak level is on days 2-6 ,which might be associated with survival of 2 months. The HSP72 level of survival groups was significant higher than that of dead groups.
