| IGF-1 is a alkalinity protein made up from 70 amino acid.It is mainly produced in liver and blood vessel smooth muscle.Now we acknowlege that IGF-1 is also present in placenta.It can mediate the invasion and move of cytotrophoblast and expansion placental vessel. IGF-1 participates in pregnancy complication.IGFBP-1 is a high-affinity IGF-1 binding protein that mainly produced by liver and deciduas.IGFBP-1 can inhibit biologic function of IGF-1 and independently adjust syncytiotrophoblast invasion.Abnormal of IGF-1 and IGFBP-1 in maternal serum and placenta induce the injury of syncytiotrophoblast invasion and placental shallow implant,On the other hand which can result in unbalanced systole and diastole of vessel that lead to injury of maternal vascular endothelium .At last,which contribute to hypertensive disorder complicating pregnancy. Objective: To measure serum and placental levels of IGF-1 and IGFBP-1 and analyse the correlation between them from patients with hypertensive disorder complicating pregnancy, as well as to investigate their role in the pathogenesis of hypertensive disorder complicating pregnancy. Methods: The Levels of IGF-1 and IGFBP-1 in serum and placental were detected from 60 patients with hypertensive disorder complicating pregnancy and 18 nomal pregnant women by enzyme linked immunosorbent assay (ELISA) and immunohistochemistry. The date is analyed by T Test, Nemenyi Test ,Linear Correlation and Spearman Bivatiate Correlation. Results: 1.Serum level of IGF-1 in hypertensive disorder complicating pregnancy group was significantly lower than that in normal pregnant group (P<0.01).There were significant different between the gestation hypertension, mind preeclampsia and severe preeclampsia groups (Pm<0.05, Ps<0.01). 2.IGFBP-1 level in the maternal serum from hypertensive disorder complicating pregnancy group was significantly higher than thatin normal pregnant women (P<0.01). There were significant different of serum IGFBP-1 level among three groups of hypertensive disorder complicating pregnancy (Pm<0.05,Ps<0.01). 3.Immunolocalization of IGF-1 and IGFBP-1 in all cases were observed mainly in syncytiotrophoblast and decidual cells.They located in cytoplasm and cell membrane. 4.The expressions of IGF-1 in the placenta of hypertensive disorder complicating pregnancy group were significantly reduced than that in normal pregnant group(P=0.002). The IGF-1 expression in mind and severe preeclampsia patients were lower than that in normal pregnancy group (P<0.05,P<0.01). There were significant different between mind and severe preeclampsia patients(P<0.05). 5 . IGFBP-1 expressions in hypertensive disorder complicating pregnancy group were higher than that in normal pregnant group(P<0.01).There were significant differences of IGFBP-1 expressions in gestational hypertension ,mind preeclampsia and severe preeclampsia groups (P<0.05). 6.The levels of IGF-1 in serum and placenta had a significantnegative relationship with that of IGFBP-1 in hypertensive disorder complicating pregnancy group(r=-0.369,P<0.01). In hypertensive disorder complicating pregnancy group and normal pregnancy group, levels of IGFBP-1 in maternal serum was positive related with that of placenta (P<0.05). Conclusions: 1.Level of IGF-1 in serum reflects the degree of hypertension disorder complicating pregnancy and can be used as a clinical surveillance target.Reduced IGF-1 in placenta may impair the ability of cytotrophoblast invasion of spiral arterioles and lead to ichemia of placenta vascular.IGF-1 can promote the release of NO and inhabit human placenta producing TXA2 åŠPGF2α,which cause unbalanced systole and diastole of vessel and the presence of hypertension disorder complicating pregnancy. 2.Level of IGFBP-1 in serum reflects the degree of hypertension disorder complicating pregnancy and can be used as a clinical surveillance target.Elevated IGFBP-1 in placenta induce the injury of syncytiotrophoblast invasion and villus blood vessel remodeling processes that result in badness of placenta implant a... |
