Objective: Type 2 diabetes is associated with significantlyaccelerated rates of macrovascular complications such as atherosclerosis.including of coronaryartery,cerebral vessels,peripheral vessels.according to statistics, atherosclerosis isabout occupied 80 percent of diabetic mortality rate,the incidence rate of coronary heart disease and stroke in diabetemillitus is 2.5 times of normal person,that of is higher in famale.but the mechanism of atherosclerosis in Type2 diabetesis not clear completely ,the risk factor such as hypertention,lipoprotein,lipid,obesity, partly explain the high incidence rate.recently,the effect of insulin-like growth factor-I (IGF-I),andIGF-binding proteins-1 in diabetic macrovascular complications,because of their unique biologic property are more and more focused on.So the aim of this present studywas to observe serum free insulin-like growth factor-I (IGF-I), IGF-bindingproteins-1 in diabetic macrovascular complications , and to evaluate their possible relationships to age,body mass index,fat distribution,glucose and lipid metablism in coronary heartdisease and diabetes millitus,explore the effect of IGF-I and IGF-binding proteins-1 in diabetic macrovascular complications. Methods: 1 Objects : ninty patients are divided into four groups: twenty-seven diabetic macrovascular complication (DMM group),twenty-five diabetes millitus without macrovascular complication (DMN group),twenty coronary heart disease ( M group ) ,eighteen normal control subjects (C group)were recruited.All objects are not injected with ectogenesis insulin before collecting blood,in order to excluding ectogenesis insulin's influence on it; the famale were not cyesis. Big vessels lesion's criteria : having cardiac angina,myocardial infarction or atheromatous plaque by Doppler Ultrasonography or High-resolution Computed Tomography. 2 Methods : recording age,sex,blood pressure(BP),values,height ,weight , circumference of waist and hip were measured to calculate body weight index(BMI),wait to hip ratio(WHR) , All patients undergone oral 75g glucose tolerance test serum glucose and insulin concentration( basal and OGTT stimulated glucose) of all patient , low density liprotein(LDL), high density liprotein (HDL) and triglyceride were measured,detecting serum IGF-1 and IGFBP-1 by ELISA. Results: Tabal 1 indicate: The weight,BMI and WHR increase significantly of DM group and M group Compared with C group (p<0.01); there was no difference of weight and BMI between DM group and M group (p>0.05);While the difference of WHR between them is significante in statistics(p<0.05);The age of four groups is no difference (p>0.05);Tabal 2 indicate: ,The level of triglyceride and low density liprotein(LDL) in DM group and M group were increased (p<0.01), but high density liprotein(HDL) decreased(p<0.01) Compared with C group;The fasting plasm glucose of DMM group was higher than that of DMN group(p<0.05); Tabal 3 indicate: There were hyperinsulinemia in DM group and M group ,the fasting, postprandial 2 hours insulin and Homa-IR scores were higher than control group(p<0.01), Compairing with control ,The level of free IGF-1 in DMM,DMN group and M group were markedly decreased (p<0.05) ; free IGF-1 level of DMM group was lower than that of DMN group (p<0.01)。The IGFBP-1's concentration were significantly lower in patients of DM group and M group than C group(p<0.05), There was no difference between diabetic patients with and without macrovascular diseases. The relationship between IGF-I, IGFBP-1and other parameters: Free IGF-I was negatively correlated with freeplasma glucose: M group (r=-0.48 p<0.05 ), DMM group (r=-0.63 p<0.001),DMN group(r=-0.41 p<0.05 );was negatively correlated with LDL (r=-0.71,r=-0.57,r=-0.57 respectively;p<0.01).and IGFBP-1 was negatively correlated with WHR (r=-0.47,r=-0.53,r=-0.62 respectively;p<0.05); was negatively correlatedwith Homa-IR (r=-0.41,r=-0.57,r=-0.53 respectively;p<0.05); The analysis of type 2 diabetes macrovascular complications risk factors by the multiple stepwise Logistic regressive anal...
