Objective To analyze the relationship of Adiponectin and the insulin resistance and cardiac and cerebral angiopathy of Type 2 diabetes.Methods Divided the diabetes patients into two groups: group B and group C. The group B was composed with the diabetic patients without macroangiopathy (n: 42) ; The group C was composed of the diabetic patients with the cardiac and cerebral vascular complication (n: 38). The patients of group C had identity cerebral infarction (according to their past history of illness, examination results from computed tomography and magnetic resonance imaging), coronary heart disease [according to their history of angina cordis, ocardial infarction, ectrocardiogram and (or) coronary arteriongraphy confirm that at least one vascular had narrow degree more than fifty percent]. At the same time, 35 examined healthy persons were enrolled in this study as control group (group A) . Their serum adiponectin (ADP) , blood glucose, blood lipid, fasting inlusin were measured. The insulin resistance index (IRI) was calculated. The carotid intima-media thickness (CIMT) was measured. The results were analyzed. Result (1) The lever of serum adiponectin of group B was lower than that of group A (P<0.01), Group C had the lowest lever of adiponectin among 3 groups (P<0.01). The patients in groupB and C had more evident metabolic disturbance and insulin resistance than group A. (2) Divide the Type 2 Diabetic to four groups by adiponectin having 25 percent, 50 percent, 75 percent percentile as the boundary.We found that carotid intima-media thickness of the four groups have significant difference with each other. (3) ADP was negatively correlated with CIMT (P<0.01). (4) Multiple linear stepwise regression analysis suggested that decrease of adiponectin and high-density lipoprotein cholesterol and the increase of triglycerides were the dangerous factors of CIMT thickening.Conclusions (1) The adiponectin level has negative correlation with carotid intima-media thickness of Type 2 diabetic. It is one of the independence risk factors which influent the carotid intima-media thickness. (2) The more serious cardio-cerebral vascular disease,the lower adiponectin levels the Type 2 diabetic have. Lower adiponectin concentration have close correlation with the development of the cardiac and cerebric vascular disease. (3) Adiponectin can protect the cardiac and cerebrovascular by improving insulin resistence, regulating glycemia and blood-fat, lowering blood pressure, slowing down the great vascular atherosclerosis of Type 2 diabetes.
