Objective: Endotoxin shock is a sort of infective shock, which is often seen in clinic and has difficulty in therapy. Infective shock is always named septic shock, the state of which expand rapidly and can result in multiorgan dysfunction syndrome(MODS). The mortality is very high in these years and the incidence of septic shock increases year after year. Although the mechanism of septic shock hasn't be clarified at present, studies have indicate that the inflammation medium cascades after endotoxin come into the bodies may be more important. Strategies using cytokine monoclonal anti-bodies such as TNFα and IL-1 anti-bodies haven't been applied successfully into cure for septic shock. Recent researches have demonstrated that propofol has effect on restraining excess inflammatory responses and is beneficial for sepsis and septic shock. This paper aims to determining the effect of propofol administered with different dose on endotoxin-induced shock in rabbits so as to providing an effective remedy for septic shock and sepsis.Material and Methods: Fifty New Zealand rabbits of either sex were randomly divided into five groups: normal control group(group A); endotoxin shock control group(group B); propofol infusion at 2 mg.kg~-1.h~-1 (group C); propofol infusion at 5 mg.kg~-1.h~-1 (group D); propofol infusion at 10 mg.kg~-1.h~-1 (group E). Group C-E received a continuous infusion of propofol 30 min after intravenous LPS. Edotoxin shock was replicated in rabbits by receiving a continuous infusion of E-coli lipopolysachaide(LPS O111B4) of 600μg/kg. The animals were observed for 5 hours after LPS. MAP, RR and HR of rabbit and arterial blood gases were recorded every hour after LPS. At the time before and 60min, 180min,300min after infusion of LPS respectively in eachanimal,blood samples were taken for late measurement of the serum CK-MB, ALT, AST, Bun, Cr, NO. At the time before and 60min, 120min, 180min, 240min, 300min after infusion of LPS respectively in each animal, blood samples were taken for late measurement of the serum TNF-a. The survival rates of the animals in the 5 groups during 48 hours after LPS were compared.Result: 1 .Infusion of LPS with 600ug/kg induced MAP of group B at the beginning. After a brevity of rising, MAP fell again. Two hours later when it came to less than 60 mmHg, the model had been copied successfully. 2.The MAP and HR gradually decreased and the RR increased in group B after LPS challenge. Compare to group B, group C, D and E significantly attenuated the decrease in MAP and HR(P<0.01). The effect of group D was better than group C and E ( P<0.01 ). Group D and E can decrease the RR and group E do the better. 3. The HCO3" and pH gradually decreased and the PaC>2 had no evidente changes in 5 hours after LPS being injected in group B. The propofol administered groups attenuated the decrease in HCO3' and pH(P<0.05, P<0.01),but group E had less effect on pH compare to other two group ( P<0.01 ) . 4. After LPS challenge, the consistence of CK-MB in serum increased rapidly, which can be controlled in group B, group CD and E( P<0.05, P<0.01). Group D do the better. 5. The ALT and AST gradually increased in group B after LPS challenge. The propofol administered groups attenuated the increase in ALT and AST( P<0.05, P<0.01). 6. The Bun and Cr gradually increased in group B after LPS challenge. Group C and D attenuated the increase in Bun and Cr ( PO.05, P<0.01 ) . 7. The contents of serum NO increased evidently from 60min after LPS challenge. Compare to baseline , the difference was remarkably ( P<0.01). Compare to group B, all the propofol administered groups can inhibit the increase in serum NO (P<0.01) . Group D did better(P<0.05, PO.01). 8. The contents of serum TNF-aof group B to E increased evidently from 60min after LPS challenge, and reached a peak level at 120 min, then gradually decreased.
