Glaucoma is a group of clinical syndromes or eye diseases that threaten the vision function of optic nerve and mainly being relative to ocular hypertension [1]. Primary glaucoma is a type of glaucoma that has nothing to do with the eye disease which has been confirmed. Its clinical manifestation is complicated [2], and the rate of causing blindness is very high. It belongs to wufengneizhang of Traditional Chinese Medicine (TCM). At present, the pathogenesis hasn't been illuminated, so there aren't effective treatments. This research based on referring to domestic and international literature and mastering the current research situation, and observed the regularity of distribution of TCM syndromes and the effect of mainly acupuncturing the acupoint of the liver meridian. We initially investigated the characteristic of TCM syndromes of primary glaucoma and introduced feasibility of acupuncture. Part one Literature researchWe summarized the cognition of name, etiology, pathogenesis, treatment of primary glaucoma in TCM from the period of Huangdineijing to tang, song, yuan, ming, qing dynasty and modern traditional Chinese physician's clinical achievements in syndrome differentiation, treatment and acupuncture, we also summarized the advanced investigation on epidemiology, etiology, pathology, pathogenesis, diagnosis and treatment and we recognized dominance and direction of modern research in domestic and international of primary glaucoma. This research wanted to provide theory support for establishing standard criterias of syndrome differentiation and exploiting external therapy which is economical, safe, convenient, effective and with characteristic of Ophthalmology of TCM. Part two Clinical Research, First The case analysis:1.Objective: To discuss the feature of TCM syndrome of primary glaucoma and to provide scientific support for syndrome differentiation and treatment in clinical work. 2.Method: We adopted retrospective investigation to register the syndrome differentiation, age, course of disease, intraocular tension and accumulated points of symptom of 114 cases (120 eyes) in the observation card of primary glaucoma. Those consequences wererelated to TCM syndromes. Whereafter, we observed the characteristic and regularity ofdistribution of TCM syndromes.3.Result:(l)Among primary glaucoma patients, syndrome of excess on liver meridian was maximum,syndrome of Qi deficiency in both heart and lung was minimum, syndromes of excess andasthenia on liver meridian were 88 cases, have 77.2% of the total amount.(2) The ratio of syndrome of excess on liver meridian in AACG was very high about 62.9%,and the ratio of syndrome of asthenia on liver meridian was low about 25.7%, the addedsum was 88.6% of the total amount; The AACG and PACG patients were all mainlysyndrome of asthenia on liver meridian, the first one was about 43.3% and the second onewas about 37.1%. The syndrome of excess on liver meridian were second, the AACGpatients occupied 31.8% and the PACG patients occupied 31.4%.(3)The comparison of course of disease between TCM syndromes of Primary glaucomashowed that the excess syndrome on liver meridian was the shortest one, asthenia on bothkidney and spleen was the longest one, between all groups' comparison there was astatistical significance. ( P<0.01or P<0.05)(4) In term of occupation on primary glaucoma patients the officer occupied the mostimportant place, intellectuals (officer and teachers etc.) are 58 cases, represent 50.9% of allpatients suffering from primary glaucoma.(5)The comparison of ocular tension of TCM syndromes of primary glaucoma showed theexcess syndrome on liver meridian was higher, and in comparison with the other threegroups the was a statistical significance( P<0.01or P<0.05). The comparison of TCMsyndromes integral showed the excess syndrome on liver meridian was higher, and incomparison with the other three groups the was a statistical significance ( P<0.01 orP<0.05).Second Clinical testl.objective: To observe the effect on ocular tension and clinical symptoms in patients withprimary glaucoma by acupuncturing taichong xingjian and hegu point of the liver meridian,and analyse the action method.2.Method: 34 patients of primary glaucoma were included in our research, and 79 eyeswere acupunctured. We recorded the patients conditions and normal result of eye investigations, and recorded the TCM syndrome, especially the ocular pression , sight andsymptoms integral. We also observed the result of acupuncture treatment.3. Result:(1)After the acupuncture treatment on patients with primary glaucoma, the ocular tensionand TCM syndrome integral were all be degraded obviously, the comparison betweenbefore and after treatment showed a statistical significance ( P<0.01). After the acupuncturetreatment on patients with primary glaucoma, the result showed a little improvement on thevisual acuity, but there was no statistical significance between before and after treatment.
