Objective: To investigate the effects of Shenfu injection on myocardial injury in patients undergoing valve replacement with cardiopulmonary bypass(CPB).Methods Forty patients who received cardiac surgery undergoing cardio-pulmonary bypass(CPB), ASA â…¡ ~ â…¢, aged 38~61y were randomly divided into two groups: group SF (Shenfu injection, n=20) and group C (control, n=20). Patients in group SF received 20ml/L of Shenfu injection in cold blood cardioplegia , while those in control group C received the same volume of 0.9% normal saline. Plasma level of cardiac troponin I (cTnI), creatine phosphokinase(CK) and creatine phosphokinase isoenzyme (CK-MB) were measured before CPB(T1), 20min after unclamping (T2), 4h after unclamping from CPB (T3) and 24h after unclamping from CPB (T4). Cardiac resuscitation and ECG were recorded..Results CK, CK-MB and cTnI level were normal before CPB and there was no significant difference between two groups. CK at T4 in group SF was lower than group C (P <0. 0.05). CK-MB at T2 and T4 in group SF were lower than group C (P <0. 0.05). cTnI at T3 and T4 in group SF were lower than group C at T2, T3,T4(P<0.05).Conclusion Shenfu injection could decrease the level of CK ,CK-MB and cTnI, and reduced the myocardial injury. Administration of Shenfu injection in cold blood crdioplegia may be benefit in clinical, and cTnI is a sensitive and specific method for the diagnosis of myocardial protection.
