Objective: To study the levels of serum and clinical significance of hepatocyte growth factor (HGF) in patients at various stages of diabetic nephropathy (DN).Methods:45 patients with type 2 DN were divided into three groups according to their urine albumin excretion rate (UAER) and glomerular filtration rate (GFR):DM1 (group of micro albumin urine): UAER 20-200μg/min, GFR>120 ml/min, 15cases. DM2 (group of clinical albumin urine): UAER>200ug/min,GFR 80-120ml/min,15cases. DM3(group of renal dysfunction): urine protein of 24 hours>0.5g/d, GFR 20-80ml/min, 15cases.The levels of serum and urine hepatocyte growth factor (HGF) were assessed by ELISA, controled with 20 cases of normal (NC) people. Fasting blood glucose (FBG), HbAlC, triglyceride (TG), cholesterol (TC), low-density lipoprotein (LDL), Lipoprotein(a) [lp(a)], renal function(BUN, Cr, GFR) , urine acide, urine protein of 24 hours, urine N-acetyl-B-D-glucosaminidase (NAG), urine ?-GT were measured in all subjects, and their correlations with serum HGF were analysed.Results:(1) The levels of serum HGF in group DM1, DM2 and DM3 were higer than those of control (p<0.01), and DM2 and DM3 were higer than DM1 (p<0.01).(2) The levels of serum lp(a) and LDL in DM1, DM2 and DM3 were higer than those of control (p<0.01), The levels of serum lp(a) and LDL in DM2 and DM3 were higer than those of DM1 (p<0.01).(3). Compared with the control group,the levels of NAG increased in 7 of 15 patients of DMl,.The levels of urine NAG in DM1 and DM2 were higer than thats of DM1,but there was no singnificant difference in three goups(p>0.05).(4) The levels of urine HGF in group DM1 and DM2 were higer than those of DM3 .There was singnificant difference between DM1, DM2 and control (p<0.05).(5) There were positive correlations between serum HGF and serum lp(a) ,LDL(r=0.456,0.349, p<0.01), and urine NAG (r=0.378, p<0.05),and there was negative correlation between serum HGF and GFR(r=0.49 p<0.01). There was no positive correlation between serum HGF and serum FBG, HbAlC , TG, cholesterol.Conclusions: (1). HGF may participate in the development of diabetic nephropathy injury.(2). lp(a) and LDL may participate in the development of diabetic nephropathy. (3). lp(a) , LDL and HGF may have counted effects in the development of diabetic nephropathy injury.
