Objective: To study the changes of liver volume and perfusion parameters in livercirrhosis with 16-slices spiral CT.Material and Methods: Precontrast CT scans were performed in 48 participants,including 28 patients with liver cirrhosis demonstrated by clinical, ultrasound andCT for liver cirrhosis group and 20 patients who were no abnormal in the liver ascontrol group. The liver cirrhosis group were divided into three subgroupdepending on their liver functions, including 7 cases in Child-Pugh A, 10 cases inChild-Pugh B, 11 cases in Child-Pugh C. Then dynamic double-section CT scanswere obtained at the level of hepatic hilum, including the liver, spleen, aorta, andportal vein by injecting 40 ml contrast material at a rate of 5 ml/s.5 minutes afterdynamic CT scan, a routine triple CT scan was performed. The hepatic arteryperfusion, portal venous perfusion, total hepatic blood flow and the hepatic arteryperfusion index, portal venous perfusion index were calculated by using maximalslope calculation. The volume of the hepatic left outlateral lobe, left inlateral lobe,right lobe and the caudate lobe were measured by using the software in the CT.Then calculated the total volume of liver and the percentage of each lobe. Byutilzing the measured value of the transverse and anteroposterior diameter of theleft, right, and caudate lobes to analyze the ratio of R1/L, R2/C. Then usingstudent-t test and one-way ANOVA to analyze the differentiation of thoseparameters, and using linear correlation to study the relationship between them.Results: The mean liver volume in the control group was 1230.74±221.78 cm3 ;The mean volume of hepatic left outlateral lobe, left inlateral lobe, right lobe andthe caudate lobe were 186.08±51.03 cm3,223.62±46.80 cm3,782.87±163.29cm3,38.15±8.10 cm3 respectively; and their percentage in the total liver volumewere 15.08±3.14%,18.29±2.99%,63.43±4.28%,3.18±0.83% respectively;The ratio of R2/C was 2.56±0.41 cm(range 2.05 cm~3.27 cm) and the ratio ofR1/L was 1.32±0.25 cm(range 1.04 cm~1.82 cm); The mean HAP, HPP ,THBFand HAI, PPI in control group was 0.3384±0.1409 ml.min-1.ml-1, 1.1063±0.3571ml.min-1.ml-1, 1.4449 ±0.4633 ml.min-1.ml-1, 23.37 ±6.12%, 76.62 ±6.12%,respectively; The changes of the hepatic volume in the cirrhotic group were: Theliver volume and the left inlateral lobe, right lobe reduced, however, the leftoutlateral lobe enlarged and the caudate lobe enlarged slightly. All of the volume ofthe hepatic lobes in cirrhotic patients and /or their ratios in the total liver volumechanged. 6 cirrhosis patients measured by the lober diameter whose R2/C and R1/Lwere normal, but their hepatic lobes volume had changed (4 patients all liver lobesshrinked symmetrically, 2 patients left outlateral lobes volume and their percentagein the total liver volume enlarged ). There was a rule of the changes of the livervolume in the Child-Pugh A,B,C cirrhotic patients: the mean total liver volumereduced (1049.42±316.20,852.93±169.51,764.15±188.59) cm3 respectively,and the change of right lobe (594.86±145.57,557.35±144.43,437.36±46.33)cm3and the left inlateral lobe(184.24±139.70,103.85±39.84,117.13±38.56)cm3was the most obvious; The left outlateral lobe enlarged , however, Child-Pugh Alarger than B and C, no difference between B and C can be founded. The changesof the perfusion parameters in the cirrhotic group were: HAP and HAI increasedbut the HPP, THBF and PPI decreased. There was a rule in the Child-Pugh A,B,C cirrhotic patients: The PVP decreased step by step (0.9403±0.1882,0.8362±0.1607,0.7365±0.1235) ml.min-1.ml-1respectively; Although the HAI increasedgradually (30.48±4.93%,35.04±5.01%,37.37±4.38%respectively), but theHAP decreased little by little(0.4714±0.1657,0.4521±0.1019,0.4401±0.8209)ml.min-1.ml-1 respectively; The THBF in Child-Pugh B and C were (1.2884±0.2315,1.1766±0.1705) ml.min-1.ml-1 respectively, which was decreased moremarkedly than Child-Pugh A(1.4148±0.3367 ml.min-1.ml-1), but the differencesbetween B and C was not obvious.
