Now plaque rupture plays a important role in acute coronary syndromes(ACS) pathogenesis, It includes unstable angina pectoris(UAP) and acute myocardial infarction(AMI) and sudden death. It was more reported that inflammation played a important role in ACS pathogenesis. In order to prove that the inflammation is existent in ACS and to evaluate the beneficial effects of Simvastatin on the inflammation. 100 patients with ACS from August 2003 to December 2004 randomly were treated with routine drugs group or addititonal Simvastatin group, After 6 and 12 weeks of treatment. We compared serum levels of C-reactive protein (CRP) and tumor necrosis factor- α (TNF- α ). Fifty-six age-matched subjects with SAP or no cardiac disease served as another control group. The results as follows:1.Variation of levels of serum CRP and TNF- α , TC and TG in patients of ACS with control group.Baseline levels of CRP and TNF- α and TC were significantly higher in patients with ACS than those in control groups. The content of triglyceride (TG) in blood was uncdifferentiated. These results showed that serum levels of makers related to inflammation and TC were hign in ACS.2.Comparison of levels of serum CRP and TNF- α , TC and TG in Simvastatin group with routine group.Levels of serum CRP and TNF- α in ACS were decreased after treatment of Simvastatin and routine, But levels of serum CRP and TNF- α were decreased more significantly in the Simvastatin group than in the routine treat group and also decreased TC and the longer treatment the more lower levels of serum CRP and TNF- α were, Simvastatin can depress inflammatory reactivity of plaque. Thus it plays important role in plaque stability. There were no influence on TG in two groups and routine group hand no influence on TC.These results showed that inflammation was existent in plaque of ACS and Simvastatin could significantly depress inflammation.
