Objective In the paper, the clinical data on 758 patients with the occupying lesions in sellar area were studied and the microanatomy of sellar region via the operative approaches was studied in the adult cadaver brains, in order to enhance the understanding of the occupying lesions, microanatomy and operative approaches to sellar region, thereby to improve our clinical works.Method 1 758 cases with sellar region lesions wereretrospectively studied about their sex, age, clinical features , imaging findings, diagnosis and differential diagnosis, histological features , treatments and complications, who were treated at the Tianjin HuanHu Hospital between 1999 to 2004.2 The anatomy lines about sellar area were measured by CT. The micro- anatomy of the nerves and blood vessels in sellar region was studied in 5 adult cadaver brains under operative microscope.Results All of 758 cases includes 27 categories according topathology, which consises of 23. 7% of total intracranial mass lesions in the same period, including 544 pituitary adenomas (71.8%), 69 craniopharyngiomas (9.1%), 42 meningiomas (5. 5%), the other lesions 103 cases (13.6%). Transsphenoidal approach is the better selection for treating mostly pituitary adenomas. Pterional approach is an ideal approach for removal of the suprasellar lesions. In the anatomy study, the lines and microsurgical space were described and measured.
