Objective Estradiol(E2) is a kind of internal gonadal hormone .It can promote tumorous' occurrence and development.The metabolism and inactivation of E2 are majored in liver,and its metabolic product have different roles in hepatocarcinogenesis.At present we have found that the concentration of estrogen receptor (ER) is concerned with hepatocarcinogenesis,and it is related to the patient's prognosis.The concrete mechanism is unknown.The urokinase type plasminogen activator(uPA) and its receptor(uPAR) are paid close attention in recent years.Its main effectiveness is participated in the activation of plasminogen and the degradation of the extracellular matrix, the cytothesis and the remodeling of tissue.It is an important factor of tumorous invasion and metastasis.This study is to investigate the expression of E2, ER, uPA and uPAR in HCC,their relation with infiltration and metastasis of HCC and their relativities.Methods The expression of E2,ER,uPA and uPAR in HCC were tested using the antibody by S-P immunohistochemistry and tissue array in 50 cases of HCC,30 cases of hepatic cirrhosis(HC) and 20 cases of liver which were collected in QianFoShan hospital from Nov,2000 to April,2004.ResuIts ?E2 was majored presented in hepatoma carcinoma cell plasm.It was found that the positive rate of E2 expression in HCC (62.0%) was not higher than in HC (56.7%) (P=0.637);In HCC,the positive rate of E2 expression in â…  a+ â…  b stage(72.2%) and in â…¡a+â…¡b stage(69.6%) were obviously higher than in â…¢a+â…¢b stage(22.2%)(/>=0.025) ;In different size of HCC,the positive rate of E2 expression in HCC which diameter > 10cm (22.2%) was lower than little diameter HCC (P=0.029);The expression rate in HCC which complicated HC(73.3%) was higher than the HCC which were not complicated HC(45%) (P=0.043) . (2)ER was majored presented in hepatoma carcinoma cell plasm. It was found that the positive rate of ER expression in HCC (44.0%) was not higher than in HC (63.3%) ,but their positive rate was higher than the normal control (15.0%) (P=0.003);There were no obviously differenc of the expression rate in HCC of different stages,size and whether complicated HC. (3)uPA was majored presented in hepatoma carcinoma cell plasm. It was found that the positive rate of uPA expression in HCC (74.0%) was higher than in HC (46.7%) and in the normal control (20.0%) (P<0.05) ;In HCC the expression rate in I a+ I b stage was 50.0%,in Ila+IIb stage was 87.0%,and in Illa+IIIb stage was 88.9%,the rate of three stages had obviously difference(/>=0.015). In the different size of HCC,the positive rate of uPA expression in HCC which diameter >10cm (88.9%) was higher than which diameter <3cm(46.2%) (JD=0.038);There were no difference in the expression rate of the HCC whether complicated HC. @uPAR was majored presented in hepatoma carcinoma cell plasm. It was found that the positive rate of uPAR expression in HCC (70.0%) and in HC(63.3%) was higher than in the normal control (15.0%) (P<0.05) ;The expression rate in Ila+IIb stage(82.6%) and in Illa+IIIb stage(88.9%) were higher than the expression rate in I a+ I b stage (44.4%)(/>=0.012); The expression rate of the HCC which was complicated with HC(83.3%) was higher than the one which was not complicated with HC(50.0%) (/>=0.043).And the expression rate in different HCC was no obviously variance. (5) E2 and uPA were correlation in expression of HCC, r=0.287,.P=0.043; There were relationship between E2 and uPAR in the expression of HCC, r=0.297,/>=0.036;And uPA and uPAR were correlation in expression of HCC, r=0.308,P=0.029; ER had no relationship with E2,uPA and uPAR in expression of HCC. Conclusion ?The excessive expression of E2 take part in the occurrence and development of HCC.It was an early factor in the occurrence of HCC. ? ER was not a key factor in thedevelopment of HCC.(3)The excessive expression of uPA and uPAR take part in the development of HCC.And they were important factor in the invasion and metastasis of HCC. @E2,uPA and uPAR take synergistic action in occurrence and development of HCC. ?The study expanded new pathway of therapy to aim directly at E2 and uPA in HCC.
