Diabetes is equal to "xiao-ke" in Chinese medicine. The patients with diabetes have many complications. Infection is one of them, which is the main cause that leads the patients to deformity or death. It has been confirmed that the immunity of type 2 diabetes is depressed markedly, which include the abnormity of T cell subsets. The study to T cell subsets in diabetes has important significance in the theory and practice. The well-balanced immunity depends on the cooperation and restriction among all kinds of immunocytes.The function of thymus is to cultivate T lymphocytes, which is the centrum of cell immunity. Because T cell subsets developed and matured in thymus and CD3 is the sign of T cell subsets, if we study the expression of molecule-CD3 in thymus we can understand the status that thymocytes developed and matured. We can know the immunity of diabetic rats. We can know the influence of Tang-shen capsule to the development and maturation of the thymocytes. CLINICAL PARTObjective: to study T lymphocytes of type 2 diabetes by flow cytometry; to probe into the positive effect of tang-shen capcule to cellular immune of type 2 diabetes; to discuss the influlence which membranaceus benefit qi to improve the immunity of diabetes; in order to provide the evidences of theory and pratice for the therapy of the diabetic complications. Methods: The case of type 2 diabetes is 77 in all; the healthy control is 30. Accordding to the symptoms of patients andthe characters of tongue image and pulse manifestations, the patients were divided into yin deficiency and interior heat; deficiency of both qi and yin; deficiency of both yin and yang. The height; weight; blood glucose; the disease progression; HbAlc and so on were examined. T-cells were determined by immunoflurescent histochemistry with flow cytometry. Observed cases were gaven tang-shen capcule which include plentiful memhranaceus extract. Other drugs, which patients take to control diabetes, are equel after and before the experiment. Three months ago, we would repeat above tests. Results: T-cell subsets in type 2 diabetes are lowered than that of heath controls and the value of CD4/CD8 is increased abnormally. The difference is remarkbale (P<0. 05). The change of T cell subsets is related with blood sugar and the disease progressions. T cells of types are difference, which yin-yang both deficiency are lower than others. The difference is significative (P<0. 05). Conclusions: (1) The cellular immunity of type 2 diabetes is abnormal obviously, which is related with blood sugar and the disease progressions. We conclude that the abnormity of cellular immune is the key factor that diabetes is infectious. (2) The improved Tang-shen capsule can boost the immunity of the diabetic patients, especially to qi-yin both deficiency. EXPERIMENTAL PARTObjective: To investigate the distribution of CD3 on the surface of thymocyte in the thymus of experimental diabetic rat by the immunofluorescence histochemistry; to probe into the influence of Tang-shen capcule to the histomorphology of thymus and the development and proliferation of thymocyte. We can kown the situations of central immune organ and the effect of tang-shen capcule to central immune organ. Methods: Diabetes was induced in rats by intraperitoneal injection of 60 mg/kg of streptozotocin. Rats were divided into five groups : Controls; untreated diabetes; diabetes treated with Tang-shen capsule (with 30 mg membranaceus oral gavage daily); diabetes treated with Tang-shen capsule (60 mg membranaceus oral gavage daily) and diabetes treated with Tangshen capsule (120 mg membranaceus oral gavage daily).All experiments were performed 8 weeks after initiation of treatment. We examined immunohistochemical of the thymus of experimental rats. Results: The water and food used by diabetic rat were far more than that of the controls; the cortex and medulla of thymus in diabetic rats are thinner than the controls. The electropositive rate of CD3 is lowered. But the weight of index; the thymus index; the amount of water consumed; the thickness of cortex and medulla and the distribution of CD3+ are improved in treated groups. Conclusions: (1) The thymus of untreated diabetes rat is disorganized and the development of thymocyte of diabetic rats is arrested. The conclusion is that hyperglyceamia can prevent thymocyte to mature. (2) Tang-shen capsule can renew the injured thymus of diabetic rats and improve the immunity of the diabetic rats.
