Renal tubulointerstitial disease is also called interstitial nephritis,a kind of renal disease mainly damage in renal tubular interstice caused by many reasons. Renal tubulointerstitial lesion includes renal tubular epithelial cell degeneration, inflammatory cell infiltration and interstitial fibrosis. Various degrees of renal tubulointerstitial lesion exist in many primary or secondary glomerulopathies. Recent researchs discover: the degree of renal tubulointerstitial lesion is correlated with renal failure, and put forward that the chronic renal tubulointerstitial lesion play an important role in the course of kidney disease, the level of renal tubulointerstitial lesion is the major histological symbol that indicates patient's renal function change in the future. Therefore research to TID's pathogenesis becomes more and more important. We studied the pathological changes, levels of ICAM-1,P-selectin, renal tubular marked proteins of serum in patients of IgA nephropathy and Lupus nephritis. The pathologic types of 32 cases of IgA nephropathy are correlated with their renal tubular interstice damage degree. The degree of renal tubulointerstitial lesion is positively correlated with renal function (P<0.01). Renal tubular marked proteins(α1-mG,RBP,β2-mG,NAG) are increasing as the damage degree of renal tubular get, they show positive correlation. They can be important signs to evaluate the development and prognosis of renal tubular interstice of IgA nephropathy. Renal tubular marked proteins might be helpful for early diagnose of IgAN.We compare lupus nephritis patients of 58 cases with 11 normal persons,ICAM-1 and P-selectin of LN patients are markedly higher than those of normal persons (P<0.05 或<0.01),and ICAM-1 and P-selectin after treatment decrease significantly(P<0.05 或<0.01), P-selectin is obviously correlated with platelet(P<0.01),which shows that ICAM-1 and P-selectin take important roles in the course of LN. P-selectin probably participates in the thrombosis of LN. ICAM-1 and P-selectin of LN patients are correlated with the urine proteins(α1-mG,RBP,β2-mG,NAG) in different levels(P<0.05 或<0.01),ICAM-1 and P-selectin are correlated with creatinine(P<0.05).These results reveal that the more serious the TIL of LN patient is ,the more serious the renal function becomeand the higher the levels of ICAM-1 and P-selectin get, and also suggest that ICAM-1 and P-selectin are important in the course of TIL of LN patient.
