Objective To observe the effect of the timing of hepatic arteryreperfusion on ultrastructure changes in intrahepatic bile ductepithelial cells of the rat with liver ischemia reperfusion injury.Methods The model of partial rat orthotopic liver ischemiareperfusion was used. Fifty four rats were randomly divided into shamoperation group(SO),initial hepatic artery reperfusion(IAR) group andinitial portal reperfusion(IPR) group. The liver tissue samples werecollected at 0.5,4,12,24 hours after ischemia reperfusion in bothIAR group and IPR group. The ultrastructure of intrahepatic bile ductepithelial cells was observed by transmission electron microscope. Themorphometric analysis on the mitochondria was processed by thecomputer image analysis system.Results (1)In group IAR and group IPR, the injury of intrahepaticbile duct epithelial cells aggravated gradually along with thereperfusion. The ultrastructure changes were observed, such asmitochondria enlarged, cristae blurred or disappeared, microvillidecreased. (2)In group IPR0.5h and group IPR2h, there was not sharperinjury of intrahepatic bile duct epithelial cells than that in groupIAR at the same time, the mitochondrial average area,average perimeterand numerical density were not significantly different betweengroups(P>0.05). On the contrary, in group IPR4h and group IPR12h, theinjury was more serious than that in group IAR at the same time.Compared with those in group IAR, in group IPR the mitochondrialaverage area and average perimeter at 4 hours and 12 hours afterischemia reperfusion increased significantly (P<0.05), the numericaldensity at 12 hours after ischemia reperfusion decreased significantly(P<0.05).Conclusion Compared with IPR, IAR can protect the ultrastructure inintrahepatic bile duct epithelial cells of the rat against ischemiareperfusion injury. The morphometric analysis on the mitochondria canbe used as an indicator of the ischemia reperfusion injury ofintrahepatic bile duct epithelial cells.
