| Objective To study the relationship between panel reactive antibody (PRA) and acute rejection after renal transplantation, to investigate the clinical implication of PRA in living related donor transplantation. Method Recipient s PRA and the specificity of HLA antibodies before and after the transplantation were detected by using micro-complement-dependent-lymphocytotoxicity test with Lambda cell tray. Results In this study, the 23 recipient's PRA was negative results before transplantation, the PRA were detected in 12 cases of cadaver kidney transplantation after operation, among them, 6 cases (50%) were positive PRA .The PRA positive rate were between 13.6% and 85% (the means at 46.6%), and HLA antibody specificities were mainly A43 A68 B42 DR1 DR4 DR7 DR8 DR9 DR11, 3 cases (25%) occurred one acute rejection with positive PRA, 1 case (8.3%) with positive PRA (PRA=85%) suffered from three acute rejection and finally, due to chronic rejection, graft dysfunction, graft was removed in fifth years post-transplantation, while among 11 cases living relate donor renal transplantation, 2 cases (18%) were positive PRA. The PRA positive rate were 22.2%,30% respectively, and HLA antibody specificities were A19 DR2 DR7 . Only 1 case (9%) occurred one acute rejection with positive PRA. There was significant difference in positive PRA and acute rejection between the groups of cadaver and living related donor renal transplantation (P<0.05). Conclusion The elevation of PRA after transplantation is correlated not only with acute rejection, but also related to chronic rejection and late graft loss. The level of PRA post-operation is very important evidence predicting acute rejection. The PRA positive rate and the ratio of acute rejection in living related donor renaltransplantation are lower then cadaver donor kidney transplantation after transplantation. Living related donor renal transplantation is better then cadaver donor kidney transplantation.
