Objective: To evaluate POSSUM and P-POSSUM scoring system in predicting the postoperative mortality and morbidity in the patients undergoing total hip arthroplasty (THA). Patients and methods: 141 patients undergoing THA over three years (from January 2002 to March 2005) were studied retrospectively using POSSUM and P-POSSUM scoring system. The predicted postoperative mortality and morbidity were compared with the observed values. There were 75 males and 66 females in this group. The average age was 63.22±14.45. The average PS and OS were 17.48±5.16 and 12.43±3.05 respectively. Results: The mean PS in the group with complication were significantly higher than that of the group without complication (19.74±6.69 VS 16.49±3.96). There was no statistically difference between the patients with complication predicted by POSSUM and the observed (48 and 43). Only 3 patients in this group died after operation. The average PS in the death group were significantly higher than that of the living group (32.33±9.87 VS 17.16±4.56). There was perfect agreement between the observed death number and the predicted calculated by P-POSSUM (3 and 4), though POSSUM overpredicted the overall mortality. Conclusion: POSSUM may be used to predict the morbidity and P-POSSUM to mortality in patients undergoing THA.
