Objective:To study serum adiponectin (ADPN) concentrations of the subjects with type 2 diabetes (DM), impaired glucose tolerance (IGT) and normal glucose tolerance (NGT). To investigate the relationship between serum ADPN and body mass index (BMI), waist hip rate (WHR), gender, blood pressure (BP), fasting plasma glucose (FPG), 2-hour plasma glucose(2hPG), fasting plasma insulin (FINS) ,insulin resistance(IR) ,insulin sensitensity index(ISI) and leptin. Make basic investigations for further understanding the roles of ADPN and leptin in the onset of DM and obesity. Working out the effect of ADPN to IR, Metabolic syndrome (MS) (including glycometabolism disorder, hypertension, dyslipoproteinemia), we would hope that a new kind of medicine made from ADPN or its derivative might come out and be put into clinical application.Methods:Subjects of three groups of DM, IGT and NGT were identified by oral glucose tolerance test (OGTT), which were applied to high risk group of diabetes mellitus. All measurements (independent variables) were taken after clinic data were collected and followed by collection of fasting blood samples and 2h blood samples during OGTT for analysis. Fasting serum ADPN and leptin was detected by Sandwich technique of immunoradiometric assay (IRMA). FINS was detected by radioimmunoassay (RIA). FPG, 2hPG, total cholesterol (TC) , triglycerides (TG) were also measured on blood samples. The subjects who suffered from coronary artery disease (CAD) were diagnosed by ECG Using ISI to evaluate the function of islet p-cell. Insulin resistance index was calculated by Homeostasis model assessment (HOMA). Correlative analysis was carried out of the serum ADPN with gender, BMI, WHR, systolic blood pressure (SBP), diastolic blood pressure (DBP), mean blood pressure (MBP), TC, TG, ISI and HOMA-IR. The relationship among these factors were analyzed by SPSS 11.5 software.Results:1, There were intergroup difference in BMI, WHR, TG, BP, FPG, 2hPG, BUN, CR among DM, IGT and NGT groups.2, There were intergroup difference in ISI and HOMA-IR among DM, IGT and NGT groups (P=0.000; P=0.000 ). HOMA-IR was increased graduallyfrom group NGT—?IGT—>DM, while ISI was decreased step by step. 3 > The concentration of ADPN was increased stepwise from group DM^IGT-^NGT. (13.27±8.39 vs 16.37±9.10 vs 25.78±14.02 mg/L, p=0.000) 4> Whether analyzed as a whole or by group, serum ADPN concentrations in men were lower than those in women, especially analyzed as a whole (P=0.002) and in DM group (P=0.003)5n Whether analyzed as a whole or by group, serum ADPN concentrations in group of which BMI ^23kg/m2 were lower than those in group of which BMI < 23kg/m2 ^specially analyzed as a whole (P=0.002) and in NGT group(P=0.028)6-. Whether analyzed as a whole or by group, Pearson relation analysis showed that ADPN levels weren't correlated with liptin, TC, SBP, DBP and MBP. When analyzed as a whole ADPN concentrations were negatively correlated with BMI, WHR, TG, FINS, FPG, 2hPG and HOMA-IR, and positively correlated with ISI but not with age. When analyzed by group, ADPN levels were only negatively correlated with WHR in DM group. ADPN levels were negatively correlated with WHR, TG, FINS, FPG and HOMA-IR and positively correlated with ISI in IGT group. ADPN levels were negatively correlated with TG, FINS and HOMA-IR and positively correlated with ISI in NGT group.7% Multiple step regression analysis showed that WHR, ISI and 2hPG were the independent factors to ADPN levels in all subjects after adjustment for all other variables in the model, while WHR were the independent factor to ADPN in DM group. WHR and ISI were the ones in IGT group, similarly, ISI and FPG were the ones in NGT group.Conclusions:1> There were no difference of ADPN concentrations among different ages. 2> Serum ADPN concentrations was different between male and female, thelevels were significantly higher in women than those in men. 3> ADPN concentrations was related with obesity intently. Visceral obesitywas associated with hypoadiponectinemia.4, ADPN concentrations were also related with glycometabolism disorder intently. The concentrations of ADPN were increased gradually form group DM—?IGT-+NGT. ADPN levels were notably decreased in patients of insulin resistance syndrome. All factors( BMI, WHR, FPG> 2hPGx HOMA-IRn TG) which are related with IR were negatively correlated with ADPN.
