Objective To investigate the relationship of B-type natriuretic peptide, Insulin-like growth factor-I with the class of cardiac function, feature of transthoracic echocardiography.Methods Our study included 78 patients with heart failure and 18 healthy people as a control group. The level of B-type natriuretic peptide was measured by fluorescence immunoassay (15min), the serum level of IGF-I was measured by ELISA..All patients received routine transthoracic echocardiography. Left ventricular end systolic diameter, left ventricular end diastolic diameter left ventricular , left ventricular posterior wall thickness, left ventricular ejection fraction were measured. EF<0.55,LVPWT≥11mm were considered abnormal.Results The level of BNP in normal control group, class Ⅰ, class Ⅱ, class Ⅲ and class Ⅳ was (41.26±11.35 ), ( 156.32±15.13 ),( 338.43±23.41 ), ( 596.37±33.22 ), ( 972.35±25.43 ) pg/ml , respectively(P<0.01).It was positively correlated with the class of heart failure, while it was negatively correlated with LVEF (r=-0.90, P<0.05) .But IGF-I level in normal control group and class Ⅰ—Ⅳ was(166.72±62.48), (158.23±55.29), (203.52±63.68), (240.68±67.29),(107.78±66.43) ng/ml, respectively.No difference in IGF-I was found between class I and normal control group.But there was significant difference between class II and class I ■> class III and class II (PO.05). IGF-I decreased significantly in class IV and showed statistical difference comparing with normal control group>. class I -—'III and class IV.The level of IGF-I was positively correlated with the LVPWT (r=0.75, PO.05) .Conclusions BNP were elevated in patients with left ventricular dysfunction and correlate with cardiac function,The level of BNP might be the screen indexes for the dingnosis of heart failure,as well as the treatment guidance of heart fialure.But the level of IGF-I was different among the different class of cardiac function. No difference in IGF-I was found between class I and normal control group.lt increased significantly in class II ——III but decreased significantly in class IV. IGF-I can evaluate the prognosis of heart failure combined with the level of BNP.The level of IGF-I was positively correlated with the LVPWT (r=0.75, P<0.05) . The level of IGF-I > LVPWT and LVEF decreased significantly. This indicated that IGF-I level may be correlated with the mechanism of left ventricular remodeling.
