Objective: To explore the relationship of the changes of the jejunum mucosal structrure with the tolerance during postoperative enteral nutrition. Methods: 30 cases of advanced gastric carcinoma patients are assigned to young group(﹤40 years old n=14)and old age group( ﹥65 years old n=16 ) in General Department of the First Affiliated Hospital of jiangxi medical college. All patients take the jejunum mucosa through light and electronic microscope during operation . At the same time, the height and width of the jejunal villus and the thickness of the jejunum mucosa are measured. All patients have undertaken fluid therapy on the first postoperative day and undertaken enteral nutrition and fluid therapy from the second postoperative day to the eighth postoperative day. The complications related to enteral nutrition, for example, abdominal pain,abdominal distention, diarrhea,nausea and vomitting,dumping stomach,are observed.these data are compared with each otherin both groups. the measurement data are analyzed with SPSS13.0 For Windows software, sepnce approximate T—test of Cochran﹠Cox both groups.The incidences of complications are statistically analyzed with Fisher exact probabilities in 2×2 table. Result: 1.The height of the jejunum villus is longer in young age group than that of old age group ( p ﹤0.01 ) .The width of the jejunum villus is shorter in young age group than that of old age group(p﹤0.01).The thickness of the jejunum mucosa is thinner in young age group than that of old age group(p﹤0.05). 2.The changes of ultrastructrure of the jejunum mucosal epithelial cell in old age group are that microvilli are rare and in disorder, mitochondrial cristaes break and dissolve.The young age group is normal in the ultrastructrure. 3.The complications related to enteral nutrition are more frequent in old age group than young age group.abdominal distention(p﹤0.01), diarrhea(p﹤0.01),nausea and vomitting(p﹤0.05), Conclusion: 1. The changes of the jejunum mucosal structrure and mitochondria in the jejunum mucosal epithelial cell can relate tolerance of the postoperative enteral nutrition. 2.The tolerance of enteral nutrition is better in young person than in old person.
