| Objective To study the effects and differences of Livial, Progynova, Premarin on osteoporosis association with femoral fracture in rats, and influnce on endometrium and breast , To balance the benefits and disadvantages of treatment and side-effect. To provide the possibility of treatment for postmenopausal osteoporotic fracture.Method ninty Sprague-Dawley(SD) female rats,aged 6-month-old,were divided into five group randomly: SHAM group,OVX group,Livial group,Progynova group and Premarin group.The osteoporotic model was established by ovariectomy except for SHAM group. The right femur was broken by operation at 8 weeks later. Livial,Progynova,Premarin tablets were given through gastromy,0.26mg/ kg /d,0.0625mg/ kg /d,0.104mg/ kg /d,immediate after operation respectively.The rats in five groups were killed at the end of2,4,6,8 weeks postoperatively respectively for study. The study index include: the bone of X-rays;the histological of bone calluses ,osteoblasts and osteoclasts,endometrium and breast; the disphyseal density of cartilage and epiphysis of right femur;the biomechanical properties of right femur.Results 1. The density of calluse of OVX group was lowest in five groups.The line of fracture was still exist in OVX group while it were unclear or disappeared in other four groups. It was best in Livial group .2. There was more number of osteoclasts and in OVX group than in other four groups and there was no obvious differences of number of osteoblasts in the five groups on the surface of osseous trabecula .Original trabecular bone changed slowly into mature trabecular bone ,part of which had been absorbed and disappeared at 8 week in OVX group. In other four groups trabecules became thicker and denser gradually. The osteoblastic Index was lowest in Progynova group ^specially at 6 week.The area of trabecula was biggest in Livial group while least in Progynova group ^specially at 4 week. 3. The diaphyseal density of cartilage in Livial, Premarin,Progynova groups were better than that of OVX group,especially in Livial group.lt was better in Livial group than in Progynova group and Premarin group. The density of epiphsis in Livial, Premarin,Progynova groups were better than that of OVX group.4.The consequence of elastic modulus > elastic deformation destructive intension of right femur in Livial, Premarin groups were better than in OVX group, while the above two index in Progynova group was better than in OVX group. 5. The hyperplasia of endometrium was the same in Progynova,Premarin groups as in SHAM group, while it was not found in Livial group after 8 weeks postfracture. 6. The hyperplasia of breast tissue in Progynova group was found, while it was not obvious in other groups.Conclusion Estrogen is good for fracture healing of postmenopausal osteoporosis; The mechanism is possibaly that estrogen may inhibit bone absorption,stimulate bone formation, reduce bone tuneover, accelerate thebone-remodeling and make the biomechanics properties better.the Livial is decomposed into estrogen, progesterone and androgen while the main component of the Progynova and the Premarin is respectively estradiol and estrone.Treatment with combined sex hormones has the better effects on fracture healing than estrogen alone. At the same time Livial would not make endometrium and breast hyperplasia. Progynova and make breast hyperplasia while it is not found in Premarin group. It has not been found about endometrium hyperplasia in Progynova and Premarin groups in morphometry at 8 week. |
