This article described the mechanism of accountability of two township health centers in Dafang County, Guizhou Province. The main mechanism of accountability of the township health centers is the hierarchical control of the government. It could regulate the contents of health service in rural China, the target and the responsibility of township health centers. But it still remains the tendency of formalization. The mechanism should be improved. At the same time, the existence of private clinics turn into the possibility of "exit' for those dissatisfied with the service of township health centers. People in rural areas lacks of the channel and ability to use the democratic way to show their unsatisfactory. Conclusion: A government's "hierarchical control" over township health centers is reinforced by the public willingness and ability to "exit" in rural China. The "voice " of public still remain very low.
