| Objective To provide anatomic basis for the application of island flap pedicled with the cutaneous branch of ilioinguinal nerve and its nutrient vessels.Methods The origin, course, distribution, anastomosis and out diameter of cutaneous branch of ilioinguinal nerve and its nutrient vessels were observed on 27 sides adult perineum and low extremity specimens perfused with red latex. The blood supply range of nutrient vessels was observed on 2 sides fresh specimens injected with 1/10 Chinese black ink.Results Ilioinguinal nerve originated from the 1st lumbar ventral rami. Its cutaneous branch perforated deep fascia from superficial inguinal ring with an external diameter of 1.89+0.47 mm under spermatic cord or round ligament of uterus. Its cutaneous branch mainly dominates the skin of scrotal superior part or greater lip of pudendum, and medial closed tip of thigh. The stem of cutaneous branch is (10.8± 0.7)cm long. In spermatic cord, testicular artery, cremasteric artery and ductus deferens artery are ilioinguinal nerve's nutrient vessels. After perforating deep fascia, the nutrient vessels of its cutaneous branch originated from ascending branch, descending branch and femoral medial branch of superficial external pudendal artery and sent many cutaneous arteriole to nourish the surrounding skin. The region, half of anterior-interior-superior of thigh and above 5cm of inguinal ligament, is dyed by black ink. Especially the region centered by cutaneous branch of ilioinguinal nerve is darkly dyed, its aera is 6cm×10cm.Conclusion The island flap could be designed with the cutaneous branch of ilioinguinal nerve and its nutrient vessels as its pedicles to repair the defect of perinei. |
