BACKGROUDSDecisions on Sanitation Reform and Development by CPC Central Committee and State Department, released at the end of 1996, states that, "Governments at all levels should bear full responsibility for public health protection and disease prevention. Essential finance should be guaranteed and invested to reinforce the establishment of disease prevention and health protection institutions and to assure the effectiveness of severe disease's prevention and control." Aiming to improve public health and efficacy of public health resources, Institution of Disease Prevention and Control, an important part of China's public health system, assumes the chief responsibility of disease monitoring and prevention throughout the country. Owing to the long-existed idea of "emphasizing treatment while ignoring prevention" and the traditional model of government-funded public health service, the efficacy of disease prevention and control institutions in our country has not been brought into full play. Especially in the year 2003, SARS, a disease originally infected in certain area, soon developed into a nation-wide crisis, whose lesson has been fully analyzed and summarized by the Chinese authority. It has become an increasingly important concern of governments at all levels to quicken the establishment of public health system, especially disease prevention and control institutions. China's disease prevention and control institutions, at the present time, include different levels of Center of Disease Control (CDC) and reorganized epidemic prevention stations. Because of its short history China's CDChas some serious problems, like inappropriate orientation and inadequate function, among which fund shortage is the most vital, leading to the unfavorable efficacy of our country's CDC. Therefore, it is of great urgency to carry out research of the mechanism of China's disease prevention and control institutions, so as to quicken its establishment of public health system.On the principle of complex system theory, microcosmic economics and prevention medicine, the project studies the cause, the degree and the mechanism of the low efficacy of China's disease prevention and control institutions according to the specific situation of our country by means of different research methods and analysis techniques, as well as solutions to these problems, so as to provide policy-making assistance for relevant government sections.MATERIALSMaterials of the present research come from literature review, in which related articles from a dozen types of professional periodicals between 1990 and 2003 have been analyzed in great detail. Relevant data have also been collected from the first and the second national investigations of sanitation service.METHODSOn the basis of abundant literature review and expert consultation, the present thesis adopts the researching method of logical analysis at different levels to discuss and deduce the direct and indirect factors that have caused the low efficacy of China's disease prevention and control institutions. The mechanism of the low efficacy has been studied quantificationally by a set of mathematical formulas and methods according to Cobb-Douglas's production function theory. The quantitative model has also been testified by materials collected secondhand.The achievements of the present thesis include the theoretical and quantitative models of the low efficacy of China's disease prevention and control institutions, which reveal the way of problem solving and thus provide assistance for China's establishment of disease prevention and control institutions.RESULTSThe theoretical model can explain the efficacy problem existing in China's disease prevention and control institutions and can give support to previous research achievements. It is scientific, acceptable and manageable.The quantitative model can explain the following questions: (1) the degree of fund shortage in disease prevention and control institutions;(2) the degree of disease prevention and control institutions' value of paid service;(3) the cause of income bulge in paid service of disease prevention and control institutions.With the analysis of the current situation and efficacy of China's disease prevention and control institutions, the present thesis establishes theoretical and quantitative models of the low efficacy of disease prevention and control institutions in our country by means of the integrity of quantitative and qualitative researches. It can provide the foundation for the full performance of China's disease prevention and control institutions, and provide constructive suggestions for the decision-making of the government.
