PrefaceIn the anti— tumor process, the cell immunity induced by T — lymphocytes plays a very important role. Especially the tumor—cell killing function induced by lytotoxic T — lymphocytes , which acts as a critical role . When different T — lymphocyte subsets present abnormality quantity and function,It will result in the disorder of the immune function and killing decrease of anti— tumor ability . As a result ,the evaluation of immune function of lung cancer patient by T — lymphocyte subset is more valuabe clinically.In 1975 Carswell first discovered tumor necrosis factor (TNF) in the serum of animals injected with Bacille Calmette — Guerin endotoxin. In 1985 , Shalaby named TNF produced by Macrophage with TNF — α and lymphotoxin preduced by T — lymphocyte with TNF — β. It has been proved that TNF—α(17kd,157aa) produced by macrophages and TNF—β (25kd,17aa) , a glucoprotein produced by activated T—lymphocytes, may have the same receptors, with which they combine and put into effect. Being a cytokine with great biologic activity, the anti — tumor and cyto-toxic functions of TNF—β were noticed initially, then its anti — viral activities were proved later. Having better anti — tumor functions than TNF — αand a great potential of beingdrugs, TNF—β is considered as one of the effective products of immunotherapy. In order to investigate the changes of TNF —βand its producing cell T—lymphocytes in the serum of patients with lung cancer, and study their clinical significance, we deter-mined the TNF—(3 levels in serum and T — lymphocyte subsets in peripheral blood of 70 patients with untreated primary lung cancer and 20 controlled healthy subjects, compared them based on lung cancer types and stages, and analyzed their change regularity and clinical significance.ObjectiveTo investigate the clinical significance and changes of serum tumor necrosis factor(KTNF—(3) and peripheral blood T — lymphocyte subset in patients with lung cancer. We offer a new theoretical evidence for clinical analysis the immunological function of patients with lung.MethodsWe extracted serum samples from 20 healthy subjects and 70 untreated patients with primary lung cancers. Changes of serum TNF—J3 detected using ELISA techniques, and T — lymphocyte subset in peripheral blood measured by the flow cytometer using bicolor factorial method in the 70 lung cancer patients were compared with those in the 20 healthy subjects.ResultsThe level of serum TNF — (3 in lung cancer patients was obviously lower than that in healthy subjects, not only in various histological types but also on different clinical phases. Tlymphocyte(CD3-f-CD19 —) which can produce TNF—(3, especially activated T lymphocyte(CD8 + CD28 + ) has the same or similar statistic tendency to TNF—(3. Marked decrease of serum activated T—lymphocyte, which is the main carrier of the cellular immunity, leads to depression of cellular immunity in patients with lung cancer. Because of the decrease of the main producer, the level of serum TNF —(3 descends in lung cancer patients, which can lead to depression ofphysiological activity of TNF—(3 and its anti—tumor capability.Conclusion1. In the peripheral blood of lung cancer patient , The level of all T— lymphocyte subsets decresses,more obviously in higher clinical phase and more malignant cases. The decrease of T — lymphocyte inevitably lead to reduction of immune fuction of patient.2. The decrease of T—lymphocyte lead to the reduation TNF — (3, which consequently result in the decrease of Anti — tumor ability and immune function .3. The leves of T —lymphocyte subsets and serum TNF —13 decrease and they have a positive correlation. They have the same similar statistic tendacy . Both have the same or similar statistic tendency to be obviously lower than that in healthy subjects, not only in various histological types but also on different clinical phases.4. The level of peripheral blood T—cell subset and serum TNF —[3 reduce in patients with lung cancer. The clinical significance of the activity study may be used as an objective index and treatment .
