Hypertensive disorder complicating pregnancy(HDCP)is a commoncomplication of pregnancy, which is a multisystem disorder characterizedby hypertension, edema and proteinuria that occurs after 20 weeks ofpregnancy. HDCP remains a leading cause of maternal and perinatalmortality and extensive morbidity. Despite considerable research ,theetiology and pathogenesis of HDCP remain an enigma until now.In the recent years, with the development of reproductive immunologyand molecular biology, Apoptosis of trophoblastic cells is found in theplacental tissues of normal pregnant women and pathology pregnancy, Itplays an important role in both textural rebuilding and functionalconsummate of placental villus tissue, disproportionate Apoptosis willlead to pathology pregnancy directly.The cellular apoptosis and necrosis are two different concepts.Apoptosis is a gene-regulated-programmed cell death. It acts as a basicphysical event, taking place in all kinds of physical and pathologicconditions. The cellular apoptosis is highly conservative, which is closelycontrolled by the gene families such as the Bcl-2 family, the Caspasefamily and the IAP family.Objective: To investigate the expression of Survivin Bcl-2 and Caspase-3in placenta tissue of HDCP.Methods:101 placentas tissues of pre-eclampsia (including 51low-grade patients and 50 high-grade patients respectively served aslow-grade group and high-grade group) and 62 placentas tissues ofnormal pregnancy (served as control group)were analyzed for Survivin,Caspase-3 and Bcl-2 expression using immunobistochemistry assay(streptavidin-biotin peroxidase method).Results: ① Expression of survivin in high-grade group (40%) wasdetected in a significant lower proportion of than that in low-gradegroup (65%) and in control group (72%)(p<0.05). ② Expression ratio ofBcl-2 is 66% in high-grade group, 75% in low-grade group and 84% incontrol group. There is no significant difference among these groups(P=0.524>0.05). ③ Expression of Caspase-3 in high-grade group(64%)and low-grade group (53%) was detected in a significant higherproportion of than that in control group (23%)(p<0.01). There is nosignificant difference between high-grade group and low-grade group(P>0.05). ④ Negative relationship between the expression of Survivinand Caspase-3 could be found with statistical significance(R=-0.208,P=0.008<0.01). ⑤ Negative relationship between the expression ofBcl-2 and Caspase-3 could be found with statistical significance(R=-0.199,P=0.011<0.05). ⑥ No relationship could be found betweenthe expression of Survivin and Bcl-2 (P=0.524 >0.05).Conclusion:Lower expression of survivin and higher expression ofcaspase-3 may plays an important role in the pathogenesis of HDCP.The role of Bcl-2 in HDCP needs to be studied on.
