The Study of Fundus Fluorescein Angiography in Senile MacularDegeneration Purpose: Senile macular degeneration (SMD) is the major causeof severe irreversible vision loss in the elderly. It is a degenerativediaease which exists in retinal pigment epithelium (RPE) and Bruchmembrane choriocapilliaris complex and retina. So there is likely to beabnomal choroidal circulation in early and advanced SMD. This studywas designed to observe the choroidal circulation change of eyes withSMD, and to further investigate the relationship of SMD with theabnormal choroidal circulation change, in order to provide valuabletheories for preventing and treating SMD in clinic. Methods: By the ophthalmological apparatus, collect the resultof fundus fluorescein angiography (FFA), according to "theclassification and grading system for SMD and drusen", SMD patientsin 75 eyes of 58 patients were divided into non-exudative (18 eyes of12 patients) and exudative SMD (57 eyes of 46 patients), and controlsin 24 eyes of 24 subjects.According as the result of optometry, SMDpatients in 75 eyes of 58 patients were divided into for groups, therewere ≤0.04, 0.05~0.2, 0.3~0.8 and≥1.00 .Entered the date intocomputer, set up data-base and used statistical method to treat with thedate. Results:1,According to the level of damaging vision, SMD patients weredivided into four groups. There were 16 eyes in eyesight under 0.04and non-exudative SMD were 6 eyes (37.5%), exudative SMD were10 eyes (62.5%). There were 18 eyes in eyesight 0.05~0.25 andnon-exudative SMD were 13 eyes (72.2%), exudative SMD were 5eyes (27.8%). There were 18 eyes in eyesight under 0.3~0.8 andnon-exudative SMD were 15 eyes (83.3%), exudative SMD were 3eyes (16.7%). There were 3 eyes in eyesight 1.0 and all of them wereexudative SMD.2,In the early phase of FFA, non-exudative SMD 37 eyes of 26patients, 14 eyes (37.8%) have extensive choroidal filling deficiency,6 eyes (16.2%) have local deficiency, while no one of 24 control eyeshave apparent deficiency. The difference between both groups issignificant (P<0.01). Except for 5 eyes with extensive retinalhemorrhage, of the rest 13 eyes with exudative SMD, 5 eyes (38.5%)have extensive choroidal filling deficiency, 3 eyes (23.0%) have localdeficiency, while no one of 24 control eyes have apparent deficiency.The difference between both groups is significant (P<0.01).3,In the later phase of FFA, non-exudative SMD 18 eyes of 12patients, there were 8 eyes which added drusen, 2 eyes (25.0%) put upreveal flouresence, 3 eyes (37.5%) put up infirmness flouresence(sheltered form pigment). 3 eyes (37.5%) drusen were dyed, thedrusen's flouresence were fade time by time but the figure waschangeless. The RPE arophy were 29 eyes of 20 patients, they wereput up strong flouresence, and then fade little by little. In theexudative SMD, there were 12 eyes of 8 patients in the developmentof SMD, 9 eyes (75.0%) took on kinds of figures which was likeflowers or wheels and etc. The figure was larger by the time.3 eyescan't be observed because of haemorrhage. 6 eyes of 4 patients werein the cicatrisation of SMD, FFA took on offwhite scar. While no oneof 24 control eyes have apparent deficiency. The difference betweenboth groups is significant (P<0.01).Conclusion: The incidence of exudative SMD was elevatorywith the increase of age. According to the level of damaging vision,SMD patients were divided into four groups, exudative SMD wasmore dangerous than non-exudative SMD. FFA can help us to studythe choroidal circulation change of eyes with SMD, and to furtherinvestigate the relationship of SMD with the abnormal choroidalcirculation change, in order to provide valuable theories for preventingand treating SMD in clinic.
