Fundus Fluorescein Angiography In Unilateral Exudative Age-related Macular Degeneration

Purpose To analyze the fundus fluorescein angiography findings in patients with unilateral exudative age-related macular degeneration (AMD), and evaluate the correlations between morphology changes and visual acuity.Methods Fundus photography and fundus fluorescein angiography (FFA) were performed in a series of 74 cases with unilateral exudative AMD. They had been classified to different stages based on the image of fudus photography and FFA. Choroidal neovascularizations were classified into subtypes based on the proportions of classic CNV, and the location of CNV had been recorded. Area measurements of individual CNV and subretinal hemorrhage were made from captured angiograms. Then to evaluate the correlations between fundus morphologic changes and visual acuity.Result The typical characteristics of exudative AMD is retina and RPE are detached from the underlying structures by serous fluid or subretinal hemorrhage .FFA examination reveal that CNV formation at macular area , fluorescein leak from CNV, subretinal hemorrhage show hypofluorescence with clearly margin.1 There was statistic difference between visual acuity of the second eyes and ages (Pearson correlation coefficient r=-(X461,P=0.000 < 0.05) , but non correlation was found between visual acuity of the affected eyes and ages (Pearson correlation coefficient r=-0.212,P=0.070 > 0.05). There was statistic difference between ages and the area of CNV(Pearson correlation coefficient r=0.455,P=0.043<0.05 ) ,but there was not correlation between ages and subretinal hemorrhage (Pearson correlation coefficient r=0.194,P=0.173>0.05)2 Visual acuity of exudative affected eyes and clinical stages. Exudative affected eyes had been classified into three stages , that is early , intermediate and late stage. The worst average visual acuity were the eyes which be classified to the late stage , the average visual acuity was 0.023±0.024, eyes which were at the intermediate stage had batter average visual cauity: 0.112±0.128 , the best average visual acuity was count from eyes which were at the early stage.3 There was statistic difference between visual acuity of exudative affected eyes and location of CNV (Spearman correlation coefficient r=0.490,P=0.000 < 0.05) . The area of typical CNV was correlated with visual acuity of exudative affected eyes (Pearson correlation coefficient r=-0.476, P=0. 029 <0.05 ) The area of subretinal hemorrhage have been shown to correspond with poorer vision (Pearson correlation coefficient r=-0.341 , p=0.014 < 0.05) .There was not correlation between visual acuity of affected eyes and the type of CNV (F= 0.276,P=0.760>0.05> .4 The average area of the eyes which had classic CNV was 5. 516±3.824mm~2,and the average area of the eyes which had part classic CNV was 4. 747±4.772 mm~2. The average area of the eyes with occult CNV wasl5.118±11.195mm~2 .There was statistical difference between the area of the three groups(F=5.280 P=0.009 < 0.05).5 There was statistical significant between the visual acuity of affected eyes and visual acuity of the second eye (Pearson X~2=69.190,P=0.000, Pearson correlation coefficient r=0.279,P=0.016). The test showed statistically significant between visual acuity of the second eyes and the area of subretinal hemorrhage(Pearson correlation coefficient r=-0.312, P =0.026<0.05). But there was not correlation between the visual acuity of the second and the area of CNV (Pearson correlation coefficient r = 0.279,P=0.221>0.05).Conclusions:1 FFA can be used to diagnose AMD and to clarify the location , range , number and characteristics pathological changes , thus provide guidance to clinical therapy.2 The location of CNV , the area of CNV and subretinal hemonhage were perhaps correlated to the visual acuity of exudative eyes.3 The types of CNV were correlated to the area of subretinal hemorrhage.4 The vision of the second eye was connected with severity of the affected eye.
