Background and ObjectiveCholedocholithiasis is a widespread disease in the clinic. The incidence of the disease has inceased year after year. Auologous there are two primary treatment of choiedocholithiasis in the clinic. The tradition treatment is open surgical procedure. This method need general anesthesia and have greater trauma, more complications and longer hospitalization. However this therapy is a straightforward undoubtedly open surgery and has been developed in the hospital proverbially. The endoscopic therapy of choledocholithiasis has benefits of quicker restored time, lightercholedocholithiasis has benefits of quicker restored time, lighter pain and shorter hospitalization compared with laparotomized treatment. With the development of endoscopic techniques, it has replaced laparotomy partially. This article aids to evaluate the adventage and shortcoming of endoscopic therapy or laparotomy by analyses of 45 cases who were caught with choledocholithiasis in our department in 2005.MethodsThe data of forty-five cases with choledochalithiasis underwent endoscopic or laparotomized therapy from January to December 2005 were retrospectively analyzed. 21 cases were treated with endoscopy, 24 cases were treated with open surgical procedure. The following data of technical success of the procedure and complications were compared between two groups and related literatures were reviewed.ResultsThe success rate of complete calculous extractive was 85.71%(18/21) in endoscopy group, complications of the procedure occurred 9.52%(2/21) in endoscopy group. The success rate of complete calculous extractive was 91.67%(22/24) inlaparotomy group, complications of the procedure occurred 12.5%(3/24) in laparotomy group.Conclusions* Endoscopic therapy and laparotomy has the advantage and shortcoming severally in the treatment of choledocholithiasis. Patients have choledocholithiasis after cholecystectomy , EST is the first choice. However Patients of choledocholithiasis with gallbladder should undergo open surgical procedure.
