| OBJECTIVE The study was conducted to determine the causative factors in the postoperative recurrence (PR) of chronic subdural hematomas (CSDHs) and review the prevention and treatment of recurrence.METHODS The clinical, radiological, and operative factors of chronic subdural hematoma (CSDH) were retrospectively analyzed in 198 patients , who were treated by one burr hole surgery between January 2000 and January 2005. We evaluated the PR rate related to personal, clinical, surgical and neuroradiological imaging variables. In addition, we reviewed the prevention and treatment of recurrence.RESULTS 179 patients were successfully treated following the initial burr hole surgery, Recurrence was noted in 16 patients after the first burr hole surgery(8.1%). In recurring cases, 13 cases were cured after the second burr hole surgery and two had third recurrence,they were cured by craniostomy.CONCLUSIONS The interval from onset of symptoms to hospitalization, hematoma density, residual air in the postoperative hematoma cavity, drainage volume are important factors for the recurrence of CSDH,so we should pay attention to these factors and select appropriate surgical procedures and postoperative management to prevent and treat this condition efficiently. |
