Objective: The purpose of this study is to based a new diagnosis means of medical radiography to detect Temporomandibular Joint disorder (TMD) disc anterior displacement through the research of the TMD diagnosis by high-resolution ultrasonography (HR-US) , and analyse the sonography of the anterior disc displacement with HR-US and clinic application value of the HR-US.Methods: The doctor from ultrasound and radiology department choosed forty patients who were from temporomandibular joint disorder clinic of the the second hospital of Tianjin Medical University, examined them with HR-US and arthrography the double blind trial. Then, they got the result separate.Results: 1.HR-US shows excellent distinctiveness (61.5%) and sensitivity (77.8%) in the diagnosis of disc anterior displacement.2.HR-US is consistent with arthrography in some extent in diagnosing anteriordisc displacement (VKaPPa=0.385, P—0.016);consistent with arthrography in some extent in diagnosing anterior disc displacement with reduction (VKappa=0.400, P= 0.011);consistent with arthrography exceedingly in diagnosing anterior disc displacement without reduction (VKapPa—0.908, P=0.000)o3.HR-US can detect the joint disc movement dynamically, and diagnose anterior disc displacement with reduction and anterior disc displacement without reduction visually and clearly.Conclusions: 1.HR-US is consistent with arthrography in some extent in diagnosing anterior disc displacement and shows good clinic application value.2. Based a diagnostic criteria of the HR-US static image in diagnosing anterior disc displacement. As a nonvasive, harmless, inexpensive, easy to generalize, readily available and possible operator-dependent technique, HR-US can be used in diagnosing anterior disc displacement as a new method.3.With HR-US dynamic test, HR-US can distinguish anterior disc displacement with reduction and anterior disc displacement without reduction exactly and it isextremely consistent with arthrography.4.Based a diagnostic standard of HR-US dynamic image in the diagnosis of disc anterior displacement. Besides the above-mentioned advantages, HR-US is easier to operated than the other kind of dynamic image, and more in favor of clinical judging.
