Objective:To evaluated the indications and efficiency of retroperitoneal laparoscopic excision of pheochromocytoma. And compare the safety between open and laparoscopic adrenalectomy for pheochromocytomas. Methods:Reviewed retroperitoneal laparoscopic excision of pheochromocytoma on 33 patients from January 2000~December 2005. which were compared with 47 patients with pheochromocytoma underwent open surgery at the same term. Results:Mean tumor size was 5.2±2.1 (2~16) cm in laparoscopic surgery group vs. 5.4±3.3 (2~21) cm in open surgery group, P>0.05. Success has been achieved in 35 cases of retroperitoneal laparoscopic adrenalectomy while 1 were converted to open operation due to bleeding at our initial stage of the laparoscopic adrenalectomy. Mean operative time was 103.1±31.3 (50~230) min. in laparoscopic surgery group vs. 155.4±42.4 (96~294) in open surgery group, P<0.001. Mean estimated blood loss was 84.2±66.9 (10~300) ml, one need blood transfusion in laparoscopic surgery group vs. 315.7±339.6 (50~2000)ml, 16 patient accepted transfusion in open surgery group, P<0.001. Mean analgesic (morphine equivalents) consumption of 10.3±9.8 (0~40) mg in laparoscopic surgery group vs. 59.8±24.7 (10~120) mg in open surgery group, P<0.001. Postoperative hospital stay was 6.9±1.4 (4~10)d. in laparoscopicsurgery group vs. 8.9±0.9 (8-1 l)d in open surgery group, PO.001. Conclusions:In xperienced hands, retropentoneal laparoscopic surgery for pheochromocytoma is feasible and safe, and does not increase the specific risks associated with pheochromocytoma surgery. Since its less traumatic and quicker recovery, retroperitoneal laparoscopic surgery should be considered as the first choice for pheochromocytoma.
