Experience successively for few decades to investigate again and a-gain and the experience summary, making resection of stomach gradually reasonably with the scope of lymph node close account sweeping, premising under radical cure, guarantee the sufferer's function and the life quantities. Currently, the surgical operation is still initial means that cures the stomach cancer, cutting off thoroughly of original focus of stomach cancer and clearing metastased the lymph node and treatment of subclinical focus were the three important part of the modern surgery treatment.Metastasis of lymph node is an important biological characteristic for stomach cancer, the Japanese scholar thinks that extended lymph node dissection can improve prognosis of patients, and make D2 standard type. A great deal of clinical data confirms that the clearing of lymph node can improve prognosis of patients, but also increased the complications and death rate, so how realize the state of lymph node, guiding the scope of surgical operation, is a hot point that the domestic and international scholar investigates over a long period of time.Materials and Methods1. Materials: To study the regular pattern of lymph node metastasis in different part of gastric carcinoma and the relationship of which biological behavior through different positive lymph node in 711 cases undergone operations beyond D2, radical cure of A orB , lymph node gotten nust be more than or equal to 15, metastasis of lymph node( + ) was analyzed retrospectively.2. Methods: All the data were confirmed by pathological professors and definited the clinical stages of gastric neoplasm under the standard of UICC in 1998.3. Statistic analyze;We chose SPSS for windows software for use.Experimental Results1. Incidences of No. 3.6 LN metastasis were highest in gastric neoplasm;followed by No. 7 .4.No. 5.8.No. l.No. 9.11.2.No. 12.10.No. 13.14.17.15. 16.18 .2. Incidences of No. 1.3.No. 3.No. 6 were highest in U,M,L separately.3. Incidences of No. 10 in region U and M were much more than which in region L, Incidences of No. 11.16.17 were almost the same in region L,M and U,while Incidences of No. 12.13.14.15 in region L and M were much more than which in region U.4. The rate of LN metastasis of No. 4.No. 7.11 increases distinctly along with the increasing of measure of focus.5. LN of inchoate gastric neoplasm metastates morely in No. 7.6 and No. 3 .4 .8, the rate of LN metastasis of No. 4.No. 10 increases distinctly along with the increasing of stage of Borrman.6. It is indicated that SLN appears regular in L;No. 6, M: No. 3 , U: No. 1 through the analysis of metastases of solitary linph node.7. The skip matastasis commonly happens in No. 7, No. 8a, the depth of soakage mostly reaches the layer of SE or low poralization .Conclusions1. Extended lymphadenectomy should be performed in the lower ^ middle ? upper third of gastric carcinoma beyond I b.2. The scope of lymphadenectomy defined by Japanese General Rules for the Gastric Cancer Study (13' edition) is sufficient. Dissection of No. 12 in some cases is necessary.3. Clearing No. 7and No. 8a together with Nl is enough for nonage gastric neoplasm.4. It is helpful to locate SLN by retrospective analysis of the rule of metastasis of solitary lymph node.5. The efficiency of D2 is interfered with frequent skip metastasis.
