Objective: Analying the regularity of lymph node metastasis in thoracic esophageal cancer and its influencing factors, to provide reference for the lymph node dissection of esophageal cancer. Methods: 177 cases of esophageal cancer,with omplete clinical pathology and follow-up data,were excluded in our department of thoracic surgery, West Branch of Anhui Provincial Hospital/Anhui Provincial Tumour Hospital,from the year of 2015 to 2016, removal of cervical esophageal cancer and adenocarcinoma of the esophagogastric junction,analyzing the relationship between lymph node metastasis and tumor pathological type, depth of invasion,differentiation degree, length of thoracic esophageal cancer and vascular invasion or not. Through the results of these statistical studies,to better guide the clinical systematic, standardized dissection of lymph nodes,which we should focus on. Results: Of the 177 cases ,there were 125 males and 52 females between the age of 18-86 years old ,Median age 65 years old.And 76(42.9%) were found to have lymph node metastasis.In the 4977 dissected lymph nodes,metastasis was identfied in 361(7.3%) lymph nodes. The rate of lymph node metastasis in thoracic esophageal carcinoma was not related to the location and length of tumor(P>0.05), while with the depth of invasion and the degree of differentiation or vascular invasion,the rate of lymph node metastasis was significant difference(P<0.05). Multivariate analysis showed that the degree of tumor differentiation and vascular / nerve invasion were independent risk factors for lymph node metastasis of esophageal carcinoma. Conciusions : It is prone to lymph node metastasis in the early stage of thoracic esophageal cancer. According to the characteristics of lymph node metastasis in thoracic esophageal carcinoma,we need to make sure that we have a standardized,comprehensive and focused lymph node excision.
