Changes Of Plasma Homocysteine, Adrenomedulin, Calcitonin-gene-related Peptide And Thromboxane In Patients With Essential Hypertension And Their Roles In Pathogenesis

OBJECTIVE: To detect the levels of plasma homocysteine(HCY), adrenomedulin(ADM), calcitonin-gene-related peptide(CGRP) and thromboxane(TXB) in essential hepertension patients and investigate whether they are involved in the pathogenesis of essential hypertension (EH) and for the theory of the primary prevention of EH.METHODS: 42 essential hypertension patients without medication were randomly enrolled as study group and devided into 3 subgroups according to hypertension grouping criteria as "2004 China guidelines for prevention and treatment of hypertension": class I group (14 patients, 7male and 7 female), class II group (14 patients, 8male and 6 female), and class III group (20 patients, 10 male and 10 female). 20 healthy volunteers (10 male and 10 female) were enrolled as controlled group. The level of plasma HCY was measured by means of fluorometric method while the levels of ADM, CGRP and TXB2were determined by the method of radioimmunity. Data were processed by F test, Q test, and correlation analysis. There was statistical difference when P<0.05 and significant statistical difference when P<0.01.RESULTS: (1) The levels of plasma HCY,ADM and TXB2 in hypertensives were higher than those of normal controls, whereas the level of plasma CGRP was lower(P<0.01). (2)The levels of HCY ,ADM and TXB2 in class I group, class II group and class III group increased sequencely, and there were statistical differences between any two of the groups (P<0.01). (3) In essential hypertension groups the plasma HCY was positively correlated with plasma ADM (r=0.941,P<0.01) and TXB2(r=0.365,p<0.05),but negtively correlated with plasma CGRP(r=-0.480,P<0.01). The plasma ADM was positively correlated with plasma TXB2(r=0.310, P<0.05) , but negtively correlated with plasma CGRP(r=-0.387,P<0.05). While CGRP had no statistical correlation with TXB2 (r=-0.205,P>0.05).CONCLUSION: In essential hypertension patients, the concentration of plasma HCY,ADM and TXB2 increased and positively correlated with the level of blood pressure;the concentration of plasma CGRP decreased negatively correlated with the level of blood pressure. The changes of the plasa HCY, TXB2 and CGRP level in essential hypertension patients suggest they are involved in the genesis and developmemt of hypertension. The succeeded advance of ADM can inhibit the growth of hypertension and has protective effct on the body.
