Malignant pleural effusion is one of the complicationsof late-stage malignant tumor.It often causes severedyspnea and disturbance of circulation,which seriouslyeffect patients' living quality.If not treated in time ,it might threaten to life.Therefore , active treatment ofmalignant pleural effusion is one of the efficient measuresof extending tumor patients' life and raising the qualityof life. However,the efficiency of traditional treatmentisn't ideal.Gendicine is a gene therapy drug that is used to treatmany kinds of cancers.It is mainly composed ofreplication-incompetent recombinant adenovirus serotype5 and human wild-type p53 tumor suppressor gene.And ittransfects wild-type p53 geneinto human tumor cells,thenthe exogenous p53 gene begins to express.It speciallyinduces the apoptosis of tumor cells to treat themalignant tumors.Now rAd-p53 is used to treat more than40 kinds of tumors and it has been proved to be safe andefficient.To evaluate the efficacy and toxicity of treatment ofmalignant pleural effusion with rAd-p53,we have done thefollowing clinical research.Methods :45 cases with malignant pleural effusion wererandomly divided into two groups:the experimental group(22cases)and the control group(23 cases).Sex,age,entityand clinical stage in both groups are not statisticallydifferent (P > 0.05).All patients were treated withthoracentesis and drawn up pleural effusion,then theexperimental group was given rAd-p53(gendicine,producedby Shenzhen SiBiono GeneTech Co.,Ltd.)1×1012VP,and thecontrol group was given DDP 60mg by intrapleuralinjection.The above dosage was given once a week,for atotal of 4 weeks. Recorded the diversity of pleuraleffusion to evaluate the efficiency,observed the qualityof life as well as toxic and side reactions. Result: Thetotal effective rate of experimental group was 77.27%,and that of control group was 47.83%,and there weresignificant differences between the two groups. Thebetter quality of life appeared in the experimentalgroup.In the experiment group 11 cases got fever,it wasabout 50% of total cases.In the contral group ,it wassimilar.But the degree of bone marrow depression ,digestive tract symptoms et al of experimental group werelewer than that of contral group. Conclusion:Injections in the thoracic cavity treatment with rAd-p53in patients for malignant pleural effusion is safe andefficient.In the short term it can relieve clinicalsymptom and improve living quality.It provides us a newmethod to cure malignant pleural effusion.
