Objective: Upper gastrointestinal hemorrhage refers to the hemorrhage of alimentary tract above the ligament of Traitz, including that of the esophagus, stomach, duodenum, upper jejunum, pancreas and biliary tract, of which the hemorrhage of stomach and duodenum takes up 80~90%. Besides the major symptoms of hematemesis and melena, serious patients usually accompany with some clinical features of acute peripheral circulatory failure and insufficiency of some important organs, etc. In recent years, some haemostatic are effective but too expensive. Although hemostusia by endoscope has been widely confirmed, at present the death rate of old people (above 60) who suffer from both the acute and massive bleeding (1/3—1/2) peptic ulcer and upper gastrointestinal hemorrhage has reached high to be 8%-20%, so internal treatment is advocated in curing such diseases. According to A Treatise on Blood Troubles written by Tang Rongchuan, "Saving a portion of blood, securing a chance of survival, " Pofessor Deng Tietao, believing that patients who suffer from hemorrhage must possess the pathology of spleen weakness, combines the four steps of treatment—to stanch bleeding, eliminate the stasis of blood, calm the blood, build the blood summarized by Tang Rongchuan—with the purpose of "stanching bleeding without accumulating stasis of blood while eliminating the stasis of blood without disturbing the blood" to carry out both cure and supplement in his treatment. Through years of clinical observation, he has refined a simple but effective prescription—Jiaoqi powder, a pulvis of 30 grams of colla corii asini, 6 grams of pseudo-ginseng powder taken once or twice everyday, which has been proved to have marked improvement in treatment during preliminary observation. This thesis aims at seeking a simple and effective way without any harmful side-effect to cure upper gastrointestinal hemorrhage through the research of Jiaoqi powder in its treatment, so as to eliminatesome bad effects of medicine such as partly ulcer, perforation, etc, caused by the currently-adopted hemostasis (haemostatic, such as vasopressin, somatostatin;hemostasis by balloon compression;hemostusia by endoscope and surgical treatment) as well as reduce the its defect of high expense and risk, and further promote its clinical application.Objects: 50 cases of Patients have been identified with the major clinical feature of hemorrhage caused by stasis of blood (one type of upper gastrointestinal hemorrhage), all coming from the out-patient department of digestion and wards on Da De Road, the Hospital of Chinese Traditional Medicine of Guangdong Province, and those in Er Shadao branch hospital.Methods: By randomized double-blind control tests, the selected 50 cases are divided equally into a treatment group and a control group. The cases treated with the medicine of famotidine plus Jiaoqi powder serve as treatment group, others with famotidine plus blankness placebo (blankness placebo) as control group.Results: The experiment was adopted randomized double-blind control tests to divide the cases into two groups equally. All of the statistics were analyzed by Package for Encyclopedia Medical Statistics (PEMS). The overall efficiency and effective rates of the treatment group were 80% and 96%, while those of the control group were 60% and 92% respectively. Both the overall efficiency and effective rates in the treatment group were higher than those in the control group(P<0. 05), which indicated that the curative effect of Jiaoqi powder combined with famotidine on the treatment of digestive ulcer with upper gastrointestinal hemorrhage was superior to that of pure famotidine.Conclusion: This preparation is a kind of pure traditional Chinese medicine with reliable effects, safe and no toxic side effects, simple in treatment, convenient to use, low in prices, easy to get and suitable to be popularized and applied. Furthermore, it fully reflects the advantages of simplicity, convenience and incorporation of traditional Chinese medicine and develops a new way for the treatment of digestive ulcer with upper gastrointestinal hemorrhage with Chinese medicine. Next step will be taken to expand the number of samples under observation and choose some western medicines of better curative effects (such as Omeprazole) as contrast to further confirm its effects, and observe thecurative effects of various endoscope on the treatment of the digestive ulcer with upper gastrointestinal massive hemorrhage compared with gastroscope homeostasis so as to better and more precisely bring the advantages of traditional Chinese medicine into full play.
