Objective: The treatment of retinoic acid(RA) and arsenic trioxide (As2O3) on the acute promyelocytic leukemia (APL) had been widely accepted by the world.However ,in the course of induction-APL with RA or As2O3,70% patients suffered from hyperleukocytosis and 5%-25% patients contracted retinoic acid syndrome (RAS).When patients were treated with As2O3, a few retinoic acid–like syndrome were reported.Some researchers named retinoic acid syndrome and retinoic acid–like syndrome as acute promyelocytic leukemia differentiation syndrome. Because of its bad progress and high mortality,its early diagnosis and effective treatment were seriously considered.However,its pathogenesis has been unclear now.Therefore,It was important to clarify the pathogenesis. The present research suggested that the hyperleukocytosis and acute promyelocytic leukemia differentiation syndrome were relative to the change of adhesion molecule on APL cells.The CD11b was the mark of APL cells-differentiation.The increase of CD11b expression was possibly a cause of the hyperleukocytosis and acute promyelocytic leukemia differentiation syndrome.Now, by using RA combined with As2O3 and daunorubicin(DNR) to treat APL ,we not only improved curative effect ,but also reduced incidence rates of the hyperleukocytosis and acute promyelocytic leukemia differentiation syndrome. Whether did combination therapy led to decrease of incidence rates of the hyperleukocytosis and acute promyelocytic leukemia differentiation syndrome?So we studyed the effects of ATRA combined with As2O3 and DNR on CD11b expression of human acute promyelocytic leukemia cell line NB4 cells and explored the role of the change of the CD11b expression in the hyperleukocytosis and acute promyelocytic leukemia differentiation syndrome.
